The aim of this study was to address the need of a HTA system for Iranian health system. Methods: This study was a needs assessment study which performed in four steps. Firstly, a comprehensive review and semi-structured interviews were undertaken to identify challenges, nature and the context of health system in Iran in which the system of HTA would be applied. Secondly, a detailed analysis of individual aspects of the Iranian healthcare system was done to realize the roots and the sources of the problems. Thirdly, a wide-range review was undertaken to understand the concept, philosophy and the aim of a HTA system as one of the potential solutions to improve Effectiveness, Efficiency, and Equity (EEE) of healthcare delivery system in Iran. Finally, the priority and extent to which the introduction of HTA to Iran would assist in achieving the objectives of the Iranian healthcare system was assessed. Results: The review and the 38 interviews with key informants identified that healthcare delivery, health insurance, and pharmaceutical systems have faced serious EEE problems. These problems are originated from both structural deficiency and healthcare policies; the policies which were based mainly on pressure rather than evidence. The review of the literature illustrated that the main aim of HTA development was to improve the EEE of the healthcare system. Nevertheless to ensure such improvement, it is necessary to consider to the context of a health system. Conclusions: Development of a HTA system in Iran would enhance the development of clinical guidelines as well as economic evaluation studies. It could also improve managing new health technologies in some ways. Nonetheless focusing on the sources of inefficiency and inequity of the health system (policy and planning) could improve the efficiency of the system more effectively and thus is more cost-effective. This could also provide an appropriate context for an effective HTA system within the health system in Iran. Keywords: Health Technology; Healthcare Systems; Efficiency; Iran}, keywords = {Health Technology,Healthcare Systems,Efficiency,Iran}, title_fa = {نظام ارزیابی فن‌آوری سلامت برای ایران؛ نیاز یا تجمل؟}, abstract_fa = {مقدمه: ورود فن‌آوری‌های نوین و گران‌قیمت، بیماری‌های مزمن و افزایش انتظارات مردم از نظام‌های سلامت در سراسر دنیا، هزینه‌های بهداشت و درمان را به میزان قابل توجهی افزایش داده است. در دهه‌های اخیر برنامه‌های ارزیابی فن‌آوری سلامت HTA (Health Technology Assessment) برای بهبود کارآمدی منابع مورد استفاده در نظام‌های سلامت کشورهای مختلف به‌طور روز‌افزونی به‌کار گرفته شده‌اند. هدف این مطالعه ارزیابی نیاز به یک نظام HTA متناسب با ویژگی‌های نظام سلامت ایران بود. روش بررسی: این پژوهش یک مطالعه‌ی نیازسنجی است که در چهار مرحله در سال 1386 انجام شده و برای انجام آن از ترکیب چند روش شامل مرور متون، مصاحبه و بررسی اسناد بالادستی کشور استفاده شده است. در مرحله‌ی اول، به‌منظور شناخت چالش‌های بخش‌های مختلف نظام سلامت ایران، مرور متون و مصاحبه با افراد کلیدی در سال 1386 هجری- شمسی انجام شد. در مرحله‌ی دوم منشا مشکلات مورد بررسی قرار گرفت. شناخت مفهوم، فلسفه، و هدف یک نظام ارزیابی فن‌آوری سلامت، به‌عنوان یکی از راه‌حل‌های ممکن برای غلبه بر چالش‌های نظام سلامت ایران در مرحله‌ی سوم مورد مطالعه قرار گرفت. در مرحله‌ی چهارم اولویت استقرار HTA برای ایران با توجه به چالش‌های فعلی آن مورد تحلیل قرار گرفت. یافته‌ها: مرور متون و 38 مصاحبه با افراد خبره نشان دادند که نظام سلامت کشور در حال حاضر در بخش‌های مختلف خود با مشکلات جدی و متعددی از نظر اثربخشی، کارآمدی و عدالت مواجه است. ریشه‌ی اصلی چالش‌های موجود در ساختار و سیاست‌گزاری نظام سلامت می‌باشد. مرور متون مربوط به HTA نشان داد که هدف از راه‌اندازی آن بهبود تصمیمات مدیران و تصمیم‌گیران نظام سلامت برای ارتقای کیفیت، کارآمدی و عدالت بوده است. نتیجه‌گیری: انجام اصلاحات و برطرف کردن منشا ناکارآمدی‌ها و بی‌عدالتی‌ها در نظام سلامت (سیاست‌ها و برنامه‌ها)، علاوه بر اینکه می‌تواند بستر مناسبی برای استقرار یک نظام HTA مؤثر باشد، می‌تواند به میزان بسیار بیش‌تری کارآمدی نظام سلامت کشور را بهبود بخشیده و بنابراین هزینه- اثربخش‌تر باشد. برای اطمینان از موفقیت چنین اصلاحاتی لازم است سیاست‌ها و برنامه‌ها را بر شواهد مبتنی نمود. واژه‌های کلیدی: فن‌آوری سلامت؛ سیستم‌های مراقبت سلامت؛ کارایی؛ ایران }, keywords_fa = {فن‌آوری سلامت,سیستم‌های مراقبت سلامت,کارایی,ایران}, url = {https://him.mui.ac.ir/article_11248.html}, eprint = {https://him.mui.ac.ir/article_11248_68fc4257d77aac44e8781fcb56c0ba1f.pdf} }
