Complete and accurate information recording in the medical records of poisoned patients can play an important role in their complete clinical coding. This study aims to determine the amount of registered information required for clinical coding in the medical records of poisoned patients admitted to Imam Reza Hospital in Mashhad, Iran.Methods: This descriptive cross-sectional study was conducted in 2019. 387 records were selected and reviewed by simple random sampling from all medical records of patients admitted to the poisoning ward of Imam Reza Hospital of Mashhad University of Medical Sciences. The instrument was a valid and reliable checklist. Data analysis was conducted using descriptive statistics.Results: The type of poisoning was registered in 387 (100%) medical records and in only 20 (5.2%) medical records the poisoning substance was an unknown drug. In 183 (47.3%) medical records, an external cause was registered for the poisoning, with 19 (4.9%) and 1 (0.3%) medical records including the individual's place of exposure and activity during exposure, respectively.Conclusion: The findings of this study can be used to plan and compile the necessary guidelines for the proper accurate documentation of medical records and the possibility of high quality clinical coding. }, keywords = {Poisoning,Medical Records,Clinical Coding,Documentation,International Classification of Diseases}, title_fa = {بررسی وضعیت ثبت اطلاعات مورد نیاز کدگذاری در پرونده‌های بیماران بستری مسموم در بیمارستان امام رضا (ع) مشهد}, abstract_fa = {مقدمه: پرونده پزشکی را می‌توان به عنوان سندی از فرایند درمان بیمار تعریف کرد. ثبت اطلاعات کامل و دقیق در فرم‌های پرونده بیماران مسموم، می‌تواند نقش بسیار مهمی در کدگذاری کامل پرونده پزشکی این بیماران داشته باشد. بنابراین، هدف از انجام پژوهش حاضر، تعیین میزان ثبت اطلاعات مورد نیاز کدگذاری در پرونده‌های بیماران بستری مسموم در بیمارستان امام رضا (ع) مشهد بود.روش‌ بررسی: این مطالعه به صورت توصیفی- مقطعی در سال 1398 انجام شد. 387 پرونده به روش نمونه‌گیری تصادفی ساده از میان تمام پرونده‌های بیماران بستری در بخش مسمومین بیمارستان امام رضا (ع) دانشگاه علوم پزشکی مشهد انتخاب و بازبینی گردید. ابزار تحقیق شامل چک‌لیستی معتبر و پایا بود. داده‌ها با استفاده از آمار توصیفی مورد تجزیه و تحلیل قرار گرفت.یافته‌ها: نوع مسمومیت در 387 پرونده (100 درصد) ثبت شده بود. تنها در 20 پرونده (2/5 درصد) برای نام ماده مسمومیت‌زا، مسمومیت دارویی نامعلوم درج شده بود. علت خارجی مسمومیت‌ها در 183 پرونده (3/47 درصد) ذکر شده بود که تنها در 19 پرونده (9/4 درصد) مکان فرد در هنگام مواجهه و در 1 پرونده (3/0 درصد) نیز فعالیت فرد در هنگام مواجهه درج شده بود.نتیجه‌گیری: نتایج به دست آمده می‌تواند در جهت برنامه‌ریزی و تدوین دستورالعمل‌های لازم برای مستندسازی صحیح پرونده‌های پزشکی و امکان کدگذاری باکیفیت به کار گرفته شود.}, keywords_fa = {مسمومیت,مدارک پزشکی,کدگذاری بالینی,مستندسازی,طبقه‌بندی بین‌المللی بیماری‌ها}, url = {https://him.mui.ac.ir/article_11747.html}, eprint = {https://him.mui.ac.ir/article_11747_fc1e770017b2c2db202e0fbff81ad1b3.pdf} }
