Instant access to individuals, health information is influential for the health promotion throughout the community in both personal and public domains. Patients’ lives endanger due to lack of timely access to their own health information and subsequently lead to adverse events. Patients’ health information fragmented due to either geographical distribution of health care organizations or lack of integrated information systems. Todays, Health information exchange systems facilitate data sharing among health care entities and improve health management through the community. This study was conducted in 2014, searching the literature using the term of “Health Information Exchange” in technical databases, including PubMed-MEDLINE and Springer during 2004-2014. Totally, 9 out of 52 retrieved articles were omitted as redundancy in both databases and irrelevancy. After that, the relevant abstracts were reviewed and their full texts were extracted. Additionally, Google search engine and especially scholar section were searched to obtain related literature, including library resources of Health Information and Management Systems Society. This review was aimed at explaining the essentials of health information exchange including the history, architectural strategies, common technical models and the advantages of being use of Health Information Exchange systems. Moreover, the barriers to the implementation of such systems were discussed.

Introduction: Patient safety is a critical component of health care quality. In addition to human suffering, unsafe health care exerts a heavy economic cost to the healthcare system. The aim of this study was to assess the Patient safety infrastructures in selected hospitals of Isfahan University of Medical Sciences.Methods: The present descriptive-analytic study was carried out cross-sectional in spring of 2013. Data were collected Using compulsory standards checklist that is used by World Health Organization (WHO) to assess patient safety in hospitals through observation, interviews and reviewing documents in selected hospitals of Isfahan . the data using SPSS 21 , descriptive and inferential statistical methods was analyzed .Then, based on the Compliance level with infrastructures of patient safety, hospitals were classified into three levels of poor (lower than 50%), medium (5o-75%), and good (higher than75%).Results: The overall compliance level of patient safety infrastructure mean with 55,3% was evaluated intermediate in studied hospital. The leadership and management with the highest mean (69.5%) was evaluated average and the patient and public involvement domain with the lowest mean (37.5%) was evaluated weak .Conclusion: In total, the overall compliance level of patient safety infrastructure was evaluated intermediate in studied hospital. Given that acquiring hundred percent of critical standards in in the four domains are essential to be considered at the basic level of patient safety friendly hospital. Therefore, Necessary investments in strategic and operational planning must be considered in order to achieve 100% standards in the four domains

On one hand, drug therapy is one of the most powerful tools for the treatment of diseases, and on the other hand, one of the most common medical errors is related to hand written prescription. Patient safety is endangered due to illegible of hand written prescription by physicians and thereby the potential of medical errors would increase. Implementation of electronic prescribing and electronic health records can solve multiple problems of hand written prescription and decrease expenses of medical errors. The aim of this study was to introduce electronic prescription and its impact on medical errors. The study was a narrative review, which was performed by searching in Google search engine, PubMed, ISI web of science, Scoupus, and Embas, with keywords such as Electronic Prescription، E-prescription، Medication Errors، Patient safety’, in journals (text and abstracts), books, proceeding of conference articles, and scientific documents from 2000 to 2014. This study was done in 2014. About 58 articles, books, theses, and proceedings were obtained of which 35 cases were selected based on their relation to this issue. E-prescribing technology is a powerful tool to manage patients' medication that can lead to improve the quality of drug treatment. This technology associated with Electronic Health Records and using the support of decision support systems (DSS) can reduce the medication errors and increase patient safety is medication.

Introduction: Since hospital information systems in our country are limited on HIS first-generation activities it is necessary that these systems inter into a new wave by emphasizing on improving quality of care.Thus, this study was performed to design the quality improvement and patient safety requirements in hospital information systems. Methods: This descriptive, sectional and applied research was done in15 hospitals affiliated to Tehran, Shahid Beheshti and Isfahan universities medical science in 2012. First, library studies and internet browsing was done and based on that, an index and a semi-strutted questionnaire were prepared and presented to 5 experts of first round. Then, based on pluralization of experts' opinions in firstround, the final questionnaire was prepared including 25 closed questions with Likret scoring, validity and reliability were confirmed through content validity and test-retest method and was presented to experts of the second round by verbal reference. Answers were graded from 0-4 and were analyzed using the software SPSS version 18 and the requirements that their average of final score was 3 and more were confirmed. Results: The mean score of all determined requirements was more than 3. All experts confirmed the need of warning in abnormal test results, %94/8 providing clinical guidelines and %94/7 providing warning in case of non-conformity of the prescribed measures with physiological parameters and providing a differential diagnosis based on clinical symptoms of the patient in hospital information system. Conclusion: All proposed requirements of the initial model were approved by experts, also some items added to it. Using this model for hospital information systems evaluation and selection are recommended. Keywords: Quality Improvement; Patient Safety; Hospital Information Systems; Clinical Governance

Introduction: Global patient safety is the concern of all areas of the health care. In addition to the humansuffering in those inflicted unsafe care and services, it has a heavy economic cost. Due to the sensitivepatient safety in the operation rooms, the present study aimed to assess patient safety in the operating rooms.Methods: This was a descriptive cross-sectional study that was conducted in Shahid Beheshti Hospital,Isfahan, Iran operating room in 2011. The data were collected through standard questionnaires for patientsafety friendly hospital census from 20 senior, middle and executive managers as well as operating roomstaff of Shahid Beheshti Hospital. In order to assess the level of patient safety, average score of 0-0.5represented a poor level of performance, 0.5-0.7 an average level and 0.7-1.0 showed an intermediate level.Results: Average score for rule and leadership was 0.7 ± 0.35. In community participation and interactionwith patients 0.56 ± 0.37, in evidence-based clinical services 0.73 ± 0.35, in safe and secure environment0.78 ± 0.37 which indicated the average, good, good, and good states, respectively. The average overallsafety score was 0.71 ± 0.36 and showed a good performance.Conclusion: Although the score for the level of safety was good, the organizational policies and programsin this area at present are not sufficient to achieve a higher level. Necessary investments in strategicplanning must be investigated in order to achieve 100% standards in the four domains

Introduction: Health care organizations, especially hospitals, must focus on patient safety as their mainstrategy. Cultural change is the most important challenge that they are faced with in creating a safesystem. The present article aims to improve the patient safety culture with an assessment of the safety inFatemeh Zahra Hospital, Najafabad, Iran.Methods: This descriptive and cross-sectional research was conducted in 2010. Patient’s safety culturewas measured using the Persian adaptation of the Hospital Survey on Patient Safety Culture. This surveywas validated by the opinion of 15 experts. The reliability of the questionnaire was determined byCronbach’s alpha and was 0.846. The questionnaire was sent to all medical employees of Fatemeh ZahraHospital. Data were analyzed with t-test and one-way ANOVA by SPSS Software. A P-value of less than0.05 was considered as statistically significant, and 95% confidence interval was calculated.Results: Overall, 196 of the 350 individuals responded to the surveys. The overall response rate was 56%.Supervisor expectations and actions gained 76% of all scores, teamwork within hospital units 75%,organizational learning 73%, feedback and communication about error 70%, transitions 69%,communication openness 68%, management support 65%, teamwork across units 62%, non-punitiveresponse to error 54%, and staffing gained 48%. 64% of all employees had no reports of error. The meanpatient safety score was 3.36 (out of 5). According to t-test results, there was no significance differencebetween patient safety scores in direct and indirect contacts with patient (P = 0.373). Moreover, accordingto the results of ANOVA, there was a significant difference between the scores of the maternitydepartment and midwifes, and other wards and employees (P < 0.000). There was no significant differencebetween the patient safety culture scores of employees of different wards, work experience in the ward(their current place of work), work experience in the hospital, and type of contact with patients (P < 0.05).Conclusion: Although the total score of patient safety culture was acceptable, non-punitive response toerror, staffing, teamwork across units, and management support are in need of immediate intervention.The hospital can improve the patient safety culture by planning and taking action in these areas.
