چکیده
مقدمه: نیاز حیاتی نظام ارایهی خدمات سلامت به اطلاعات و عدم پاسخگویی پروندههای کاغذی به دلیل محدودیتهای ذاتی آن، باعث حرکت به سوی سیستمهای اطلاعات کامپیوتری شد که آرمان و هدف نهایی این سیستمها، دستیابی به پروندهی الکترونیک سلامت (HER یا Electronic health records) میباشد. اما پیادهسازی آن در سازمانهای مراقبت سلامت یک کار دشوار و پیچیده است و با محدودیتها و موانع اجرایی مواجه میباشد. هدف این تحقیق، شناسایی موانع اجرایی EHR از دیدگاه ارایه کنندگان خدمات سلامت و ارایهی راهکارهایی جهت رفع آن بود.
روش بررسی: این مطالعه از نوع توصیفی- مقطعی بود که در سال 1388 انجام گرفت. جامعهی پژوهش شامل ارایه کنندگان خدمات سلامت (از طبقات مختلف شغلی پزشکی، پرستاری، مدارک پزشکی، علوم آزمایشگاهی و رادیولوژی) در مراکز آموزشی درمانی دانشگاه علوم پزشکی ارومیه بود که به روش نمونهگیری خوشهای چند مرحلهای، 120 نفر از آنها انتخاب شدند. دادهها از طریق یک پرسشنامه که روایی و پایایی آن مورد تأیید قرار گرفت، جمعآوری گردید. اولویتبندی موانع اجرایی از طریق معیار لیکرت و از نوع 5 گزینهای در نظر گرفته شد. اطلاعات توسط نرمافزار آماری SPSS و با استفاده از آزمونهای کای دو، t-test و pearson تحلیل شد.
یافتهها: بیشترین درصد امتیاز کسب شده در موانع اجرایی پیادهسازی EHR به ترتیب مربوط به محدودیتهای نگرشی رفتاری افراد (4/93 درصد) و محدودیتهای تغییرات سازمانی (4/88 درصد) و کمترین درصد مربوط به محدودیتهای هزینهای (8/72 درصد) بود. نتایج تحلیل ارتباط بین متغیرها بیانگر وجود ارتباط معنیدار بین شغل و محدودیت تغییرات سازمانی (003/0 = P) و همچنین بین سابقهی کاری با محدودیتهای نگرشی رفتاری افراد (006/0 = P) بود.
نتیجهگیری: با توجه به اینکه مهمترین موانع اجرایی اصلی در پیادهسازی EHR، محدودیتهای نگرشی و رفتاری افراد و تغییرات سازمان است، ضروری به نظر میرسد برای تضمین اینکه ارایه کنندگان مراقبت سلامت نگرش مناسب داشته باشند، توجه به آموزش و مشارکت آنها حیاتی است. به طوری که اگر کاربران سیستم، از ویژگیها، اهداف، مزایا و تأثیر مثبت EHR بر کار خودشان آگاهی لازم را داشته باشند و از سوی دیگر استانداردهای مربوط به محرمانگی و امنیت دادهها و همچنین قابلیت تبادل پیام رعایت شود، مقاومت آنها نسبت به تغییرات کاهش و علاقه و آمادگی آنها برای پذیرش EHR افزایش مییابد.

چکیده [English]
Introduction: The critical dependence of healthcare services systems on information along with
the indigenous restriction of paper documents in satisfying this need has caused a trend toward
computer information systems. The main goal of such systems is to achieve electronic health
records (EHR). However, implementation of EHR in healthcare organizations is difficult and
complicated. This research aimed to assess the barriers of EHR implementation.
Methods: This descriptive, cross-sectional study was conducted in 2010. It included 120
employees (such as physicians, nurses, laboratory and radiology and medical records departments
staff) from teaching hospitals affiliated to Urmia University of Medical Sciences. Data was
collected by a self-developed questionnaire whose reliability and validity had been measured by
specialists and internal consistency method (r = 0.86), respectively. The collected data was
analyzed using descriptive statistics and analytical statistics (t-test and chi-square test) in SPSS.
Results: Based on our results, attitudinal-behavioral (93.4%) and organizational change barriers
(88.4%) scored the highest among the barriers of HER implementation. In addition, minimum
scores were related to financial barriers (72.8%). Correlation analysis showed a significant
correlation between job and organizational change barriers (p = 0.003), and also between the
history and attitudinal-behavioral barriers (p = 0.006).
Conclusion: Since the most important barriers of EHR implementation were attitudinalbehavioral barriers and organizational change barriers, educational interventions seem necessary
to create an appropriate attitude among health care providers. Increasing knowledge of system
users about the features, objectives, benefits and positive effects of the system while ensuring the
confidentiality and security of HER would decrease change resistance and increase the
acceptance and participation in EHR implementation.
