Introduction: Anesthesia information about pre anesthesia events demonstrates the patients’ status and care provided to them. It can be used as a source of detection of diseases and legal judgments. This study aimed at assessing quality of anesthetic records of patients admitted to the operating rooms of Besat hospital of Sanandaj. Methods: This descriptive study evaluate anesthesia records of patients that undergone operation in Besat hospital of Sanandaj in 2011. Four hundred records selected by systematic random sampling and quality of recording anesthesia data’s in patients records were evaluated by using a checklist. Survey data’s were analyzed using SPSS software and descriptive statistics including frequency, ratio and mean. Results: The results showed that basic patient information in the 10.7%, preoperative diagnosis and suggested operation in 34.4%, preoperative medication in 22%, operation time in 100% and used anesthesia drugs in 14.5% of cases were not listed. Information about the complications of anesthesia was recorded only in 2% of forms and name and volume of fluid intake wasn’t totally recorded in 42.8% of forms. Conclusion: Information contained in the anesthesia forms of patients who are undergone surgery in Besat hospital of Sanandaj is incomplete. Evaluating the causes of defects and attempt to resolve the causes can help to improve medical education, and ability of documents files with the goals of education, research and legal. Introduction: Since hospital information systems in our country are limited on HIS first-generation activities it is necessary that these systems inter into a new wave by emphasizing on improving quality of care.Thus, this study was performed to design the quality improvement and patient safety requirements in hospital information systems. Methods: This descriptive, sectional and applied research was done in15 hospitals affiliated to Tehran, Shahid Beheshti and Isfahan universities medical science in 2012. First, library studies and internet browsing was done and based on that, an index and a semi-strutted questionnaire were prepared and presented to 5 experts of first round. Then, based on pluralization of experts' opinions in firstround, the final questionnaire was prepared including 25 closed questions with Likret scoring, validity and reliability were confirmed through content validity and test-retest method and was presented to experts of the second round by verbal reference. Answers were graded from 0-4 and were analyzed using the software SPSS version 18 and the requirements that their average of final score was 3 and more were confirmed. Results: The mean score of all determined requirements was more than 3. All experts confirmed the need of warning in abnormal test results, %94/8 providing clinical guidelines and %94/7 providing warning in case of non-conformity of the prescribed measures with physiological parameters and providing a differential diagnosis based on clinical symptoms of the patient in hospital information system. Conclusion: All proposed requirements of the initial model were approved by experts, also some items added to it. Introduction: Happiness is one of the most important concepts in their everyday people life. The aim of this study was to survey the amount of happiness among students in medical records and health information technology in School of Paramedical Sciences of Ahvaz Jondishapur University of Medical Sciences. Methods: This descriptive research was conducted in the half of 2011. The population of study was includes Students of medical records and health information technology in Ahvaz Jondishapur University of Medical Sciencess. 91 students were selected randomly. The Oxford Happiness Questionnaire was used to measure happiness, Validity and reliability had been confirmed in previous studies. The collected data were analyzed using descriptive statistics in SPSS (version 15) software. Result: The results indicated there is no statistically significant relationship between the happiness and factors such as education, gender, marital status, age, field of Education, and course and semester(p<.05), but as description statistic the male students were happier than female students, married students happier than single students and younger student happier than older students. Also, with increase the number of semester we've seen a decline in student happiness. Conclusion: The results indicated that the married students were happier than single students. In explaining this finding, it can be said that there is relationship between happiness and feel of belong.. Introduction: Evaluation of scientific output of research centers could help in recognizing active centers, plus establishing a guideline in fair distribution of budget and human resources. The goal of this study is evaluation of scientific output of research centers through identifying scientometrics indices in Scopus citation index. Methods: This study has been done in a cross-sectional descriptive survey. Data has been collected through direct observation and all indexed documents of shiraz medical university (with research center affiliation) in scopus has been surveyed by the end of 2010 and five scientometrics indices was calculated for them . Results:"Medicinal and Natural Products Chemistry" Research Center has the most number of documents (169) and citations (589). The highest h-index and g index is for "shiraz institute for cancer research" and "hematology" research center and "Medicinal and Natural Products Chemistry" Research Center (h=11, g=17). The highest R-index also is for "Hematology" Research Center” with 16 value and and highest A-index (with the value 25.75) for "Gastroenterohepatology" Research Center. The most value for m-index is for "Medicinal and Natural Products Chemistry" Research Center ( m=1.571). Conclusion: Comprehensive evaluation could be done with calculating more than one scientometric indices. also the amount value of the A-index and R-index are the same for all the research centers. Introduction: By studying and identifying the psychological characteristics of librarians’ entrepreneurship, we can think up innovations in accordance with informational needs of the clients and provide them with better service in the shadows of entrepreneurial activities. The aim of the research is to determine the motivational (psychological) characteristics of the entrepreneurship of the librarians of the medical libraries of Islamic Azad University (IAU) on the basis of psychological characteristics of the Humanistic school of thought and the correlation of the demographic variables with them and the effect of those variables on them. Methods: The method of This research was analytical survey. The participants of the study were 98 librarians of the medical libraries of Islamic Azad University in 2012. The data were gathered through interviews done with experts in order to select the motivational (psychological) characteristics of the entrepreneurship and through a researcher-made questionnaire for measuring those characteristics in the librarians. The descriptive and the inferential statistics (KMO and Bartlett’s test, one-way ANOVA, correlation coefficients and determination coefficient) and SPSS12 were used for data analyzing. Results: The motivational (psychological) characteristics of the entrepreneurship of the librarians of the medical libraries of Islamic Azad University (IAU) were significantly more than average. The educational level and library management degree were significantly correlated with the motivational (psychological) characteristics. Moreover, the coefficients of correlation between the variables of academic degree and educational level were positive and direct, and with an increase in the value of those variables, the motivational (psychological) characteristics of entrepreneurship arose. Conclusion: It is expected that the librarians identify the grounds for the entrepreneurial activities in libraries because libraries, with their scientific nature are ideal lacunae for making use of creativity and innovation in economic issues, and there still exists a valuable and sellable commodity named information. Information is the currency in the new economy and taking advantage of it needs employing librarians with entrepreneurial characteristics. Keywords: Entrepreneurship; Libraries, Medical; Librarians.

Introduction: Indexing is a new way of organizing information which due to the spread of information and the ducts need to increase the speed and accuracy of produced information is important. The present study aimed to investigate the familiarity and use of indexes in the end of the book between academic members of Isfahan city based on Innovation Diffusion Theory (IDT). Methods: This study was conducted through a descriptive survey design. Study population included 1727 academic members of governmental universities in Isfahan. Sample size was calculated based on Cochran formula as 176 subjects. With respect to questionnaire return drop, the questionnaires were sent 300 subjects who returned 191 questionnaires. Questionnaire validity was confirmed by 10 experts in knowledge and informatics science, informatics and communications and sociology. Its reliability was confirmed by Cronbach's alpha=97.6. The data were analyzed by descriptive (frequency, percent and mean) and inferential (Spearman correlation Coefficient) statistical tests. Results: The findings showed that familiarity and use of academic members in Isfahan with indexes in the end of the book was lower than average (2.37). Most familiarity was through university and the use of books in Persian and Latin icons in the second priority. Results showed no significant difference between mean score of academic members' use of indexes in searching the information in the books, and sociological variables. There was an inverse association between academic members' use of indexes in writing, translating (kind of print) and age (p-value=0.019, r=-0.308). There was an inverse association between academic members' use of indexes generally and years of teaching (p-value=0.023, r=0-.296) Conclusion: Since the main mission of faculty members is teaching and research activities and performance it need understanding research tools, it seems index is the most important. Small knowledge and limited use of index among academic members of governmental universities in Isfahan may get the opportunity to practice deep. Therefore, the scientific community need professionals in this field and this can interfere with the librarians to compensate. Introduction: Evaluation of hospital performance with respect to the specific role of hospitals in health care in any country is very important. Effective and efficient operation of the hospital or in other words practice and proper use of resources will be evaluated using performance indicators. In the present study has been examined the relationship between hospital performance indicators and the degree evaluation. Methods: This cross-sectional descriptive study was performed in 2010-2011. The study sample included 22 hospitals affiliated to MUMS except psychiatric hospital Ibn Sina Hejazi. Data used in this study include standardized indicators of hospital activity which were collected by check list of statistical automation system of Mashhad University of Medical Sciences. Accuracy of information was approved by experts of bureau of University Statistics. To investigate the relationship between performance indicators and evaluating degree the hospitals, Spearman and Kendall's rank correlation and t student were used. Data were analyzed using the SPSS software. P<0.05 was set as the level of significance. Results: The evaluation was conducted in 2011, %72.7 of hospitals had grade 1 and %27.3 had grade 2. In evaluation in 2011, average bed occupancy rate, length of stay for patients, mortality rate, bed turnover rate and bed turnover interval were 69.80±50.15%, 2.67±1.16 days, 15.37±10.39 per thousand, 116.68±50.15 times a year and 1.14±0.53 days, respectively. %50 of hospitals in terms of bed occupancy rate, %77.3 in indicator length of stay for patients, %72.7 in indicator mortality rate, %100 in indicator bed turnover rate and %90.9 in indicator bed turnover interval year 2011 were an optimal status. The relationship between performance indicators and evaluating degree the hospitals significant but Intensity relationship was weak evaluated. Conclusion: Performance indicators average length of stay for patients, mortality rate, bed turnover rate and bed turnover interval were optimal and bed occupancy rate index was moderate. Weak correlation between performance indicators and degree the hospitals showed that the hospital evaluation system should be reviewed. Special the evaluation of hospital emphasis on performance indicators and evaluation conducted by an independent organization have such approaches have become more realistic evaluation of the hospital. Introduction: although Iran was fortieth country in the world that Prehospital emergency system service established in it, quality and quantity only therapeutic free service have neglected by planners and managers. This study has been done by aim of determining the relation between Prehospital time indices and on-scene mortality rate in traffic accidents in the 22 regions of Tehran. Methods: this is a correlation study depending on previous studies. The statistical Populations are all traffic accidents peoples from March 2011 till March 2012 who injured in Tehran regions. The number of studied profiles is 68335 cases. Data were mined by an information form from patient's profiles. Data were analyzed by Spearman Correlation Coefficient statistical test by SPSS16 statistical software. Results: 56.5% traffic accident patients' were carried to hospitals, 38.5% were left accident site before ambulance receiving or don’t have coordinated and 0.4% were died in accident site. The medium of age of studied patients was 35.5 years old. Response interval for 50% of mission's is less than 15 minutes and Delay interval for 63% of missions is less than 8 minutes. Conclusion: there is relation between some of Prehospital time indices and traffic accident patient’s death rate at accident site. Accuracy planning in Prehospital emergency system can promote indices and decrease death rates in this system. Key words: Prehospital Emergency Care; Indicators; Mortality; Accidents, Traffic

Introduction: In most countries, especially developing countries, hospital as an organization is very expensive. Therefore, the need for careful review and evaluation of their performance is indispensable. There are several approaches to assess the performance of hospitals, that one of the most useful is Pabon Lasso Model. In this Model and its corresponding graph three indices (Bed occupancy rate, bed turnover rates and mean hospital stay) are used. In this study, efforts have been to evaluated Performance of Qom and Kashan University of Medical Sciences hospitals by using this model. Method: This is an analytic study and in order to assess to the performance of hospitals, values of the indices-During the three years period (2009-2011) from each hospital separately was calculated. Then According to the relationship between these indicators, And simultaneous guide to Hospital Performance status using Pabon Lasso Model carried. Results: With design of graph and check hospital status on them, was observed in 2009 only one hospital from hospitals that were assessed, was with optimal efficiency (Located in district 3). In the 2010 and 2011, the total number of hospitals that were studied in two hospitals in the district is favorable and other hospitals located in other areas, Need to review the performance and are improving efficiency indicators. Conclusion: Pabon Lasso is a tool that using it in addition, comparison of the important indicators of hospital Also they can be used to evaluate the performance and efficiency. This Model enables hospital management to analyze the position of hospital and can it be a useful guide for the future planning. Introduction: The medical waste productions increases in the recent decades because of hospitals development, health care equipment's promotion ,and applying syringe. These residues' unsuitable waste disposal is the important threat for the environment health. Nurses' Performance in hospitals, which is important in medical waste disposal, is an issue that needs to be investigated. Methods: This is a descriptive- applied across correlation study was performed on 320 cases of surgical nursing staff in hospitals in Isfahan in 2012. The data were collected by researcher-made questionnaire. questionnaire which included two parts; the first part contains demographic characters of the cases and the second part contains some questions about waste disposal. Questionnaire’s content and face validity were confirmed by 10 academic members, and reliability were examined in pilot study (chronbach alpha= 0/7). Data were analyzed in SPSS software (16) by using independent t-test, Pearson correlation, one-way ANOVA analysis. Results: The research data showed no significant relation in the different nurses' operation in separating hospital waste (T test p=0.1). Also, there was no significant difference among separating condition in different shifts (ANNOVA p=0/85). In separating hospital waste, the average was 74.04, and variance was 20.39 and in collecting the average was 80.04 and variance was 13.73. The coefficient is showing no significant relation between the age and job records, but the relation between education hours and separation degree was significant. In the controlling part of the supervisors' management on evacuating residues, the scores are 74.4 for separation part, packing (73.2), collecting (78.8), transporting (95) which are showing the upper than average level in all parts. Conclusion: Performance status score of nurses in the separation and collection of relevant test under section there was no significant difference between men and women. Also the separation of state and collect the relevant test between male and female nurses working in different shifts, age and years of work and study, there was no significant association. However, the separation and collection of the hours of training, there was a significant relationship between staff nurses Scores on the regulatory status of Supervisors resolution, packing, collection, transportation represents the average is higher than average (50%) and the shipping was 95. Based on the findings and results of studies on the training of nurses in the management of medical wastes require special training program annually And create a framework that can be used for this training as teamwork, face-to-face, video, announcements, brochures, pamphlets and training groups is in the hospital. Despite the changes that have been made in recent years in the field of health, health systems Experts believe that the development of management is the major challenges of health systems. Improving the management of health organizations, have different approaches, such as quality management, health Dynamic networks, management effective program and network comprehensive has been test. But the basic approach and the missing link in this vicious cycle namely insist on Physician-centered process in the central management of health and care system, have neglected from the practitioner's point of view, researchers and policy makers. In the developing countries it is not interest to contribute to professional managers to manage health care organization. Hence, It seems that one of the most important challenges of the current health care system, be lack of fitness and accountability and a lack of structured system of manager choice in this realm. This is a narrative review that attempts with review of existing evidences and expert opinion in the field of health systems to Coordinate Physician-centered issue for informing policy makers of the country's macroeconomic management, the need for extreme care in regard to the issue management system and its emphasis. The findings suggest that our current health care system is Suffering of Excessive rust physician governance and to grow and overcome to the its major problem, need to make major changes in your current management system. Accordingly, it seems achieving to the goal of improving the management of Physician-centered health care system needs to factors such as Formulation, implementation and evaluation of pilot projects, policy development for the pilot results, capacity building, education and empowerment.
