Introduction: System security includes a set of security protections related to software, hardware, personnel and enterprise policies that protect Information Systems (IS) against internal and external threats. The present study aimed to define a comprehensive security model and then, assess hospital information systems (HIS) security in three areas of administrative, physical and technical safeguards. Methods: This was a qualitative-descriptive study. The study population included 4 public educational hospitals from different regions of the country, each with a different HIS. The data collection tool was a checklist of 134 questions. In order to design a checklist, first, the security criteria were identified from the security standards. Then, HIS security requirements were determined in three areas of administrative, physical and technical safeguards through modified Delphi method. The answers to the questions of the checklist were defined as Yes “1” or No “0”. HIS security level was identified in a five-level scale ranging from very low (0%) to very high (100%). Data were analyzed by descriptive statistics such as frequency and percentage. Results: Administrative safeguards of HIS in studied hospitals with 31.8 % and physical safeguards with 25% had a low level of security. Moreover, technical safeguards of HIS in hospitals were observed to be a medium level of security with 42.6%.Conclusion: The findings of this study expose HIS security weaknesses thus providing a good basis for managers of health information management and information technology departments in hospitals to implement appropriate corrective actions in policy formulation, user training, access control and risk management, and other dimensions of managerial and physical standards.

Introduction: The full and timely documentation of medical records plays an important role in the improvement of medical, legal, educational, treatment and research processes. Incomplete record keeping is indicative of defective care and treatment. The purpose of this study was to determine the effect of implementation of the instructions on filling the patients' records by physicians on the incompleteness of medical records of Ziaeian Hospital, Tehran, Iran. Methods: This descriptive applied study was carried out at Ziaeian Hospital in 2016. The study population included 2413 medical files filled out in August and December. The sample included 324 incomplete medical files from August (before the implementation) and 288 incomplete medical records in December (after implementation). The medical record checklist was used to collect data. SPSS and Wilcoxon signed-rank tests were employed for data analysis. Results: The mean number of defects was 12.4 and 9.9 before and 9.9 after the intervention, respectively. Number of defects in the admission document (P = 0.012), counseling request (P = 0.048), surgery report (P = 0.001), case summary (P = 0.001) and anesthesia (P = 0.012) showed a significant reduction in the number of defects and the effectiveness of the intervention. However, in total medical record documentation, the intervention was not effective (P = 0.238).Conclusion: It appears that reducing the defects in medical records of Ziaeian Hospital has not been significant which can affect the treatment of patients, the quality of transmission of information and also use of medical records by legal authorities. The results of this research can be useful for hospital directors and managers for a more optimal planning approach.

Introduction: Hospital websites have a high potential for providing information and services to customers. The assessment and comparison of hospital websites determine the extent of their utilization and contributes to the formation of constructive competition between hospitals. Therefore, the present study was conducted with the aim of comparative assessment of the websites of public and private hospitals. Methods: This applied research is an analytical-descriptive study and was conducted in 2017. The study population included 13 public hospital websites and 3 private hospital websites in Isfahan, Iran. The data were gathered through a checklist based on 11 main and 69 sub-criteria and analyzed by inferential statistics. Results: There was a significant difference between the average of scores obtained from public and private hospitals’ websites in terms of the 4 main criteria including contact information and access methods, admission service and reservation, features, design and updating of pages and public relations. However, there was no significant difference between the average of scores obtained in terms of 7 other criteria and the overall score. Conclusion: The assessed websites did not fully utilize web capabilities and did not have a favorable status. Therefore, according to the results of this research, the authorities in this area must provide the information and services to the customers in a desirable way so that they can compete with other hospitals in order to attract more customers and improve customer satisfaction.

Introduction: Due to the importance of the emergency department of hospitals, section three of health sector evolution plan emphasizes on the specialist residency program in governmental hospitals. The purpose of this study was the comparison of performance indices of Emergency Departments before and after implementation of specialist residency program under the health sector evolution plan in the hospitals of Bushehr University of Medical Sciences, Iran. Methods: This was a descriptive-analytic study performed at the Bushehr University of Medical Sciences public hospitals. The researcher collected performance indicators of the emergency ward during 2013 and 2014 period by visiting the university’s deputy of health. Data were analyzed using paired t-test and differences were presented descriptively. Results: The result of comparing performance indicators using paired t-test showed that the difference observed in performance indicators including the percentage of patients who processed in 6 hours, the percentage of patients discharged from the department within 12 hours, patients discharged against medical advice and the average duration of triage in emergency was significant (P < 0.050). While unsuccessful cardiopulmonary resuscitation (CPR) percentage showed no significant changes. Conclusion: Instructions in healthcare evolution plan for specialist residency in public hospitals were successful in improving performance indicators of emergency ward except for the unsuccessful CPR percentage which showed no significant change. In general, this plan has improved the performance indicators of emergency ward and, if continued and continuously improved, can lead to the improvement of other hospital indicators and ultimately the improvement of healthcare as a whole.

Introduction: Providing medicine as a strategic product has a special importance in every country. This research was done with the goal of managing inventories in the medicine supply chain using system dynamic approach. Methods: The current research was carried out as a case study in Mashhad Razavi Hospital, Iran. Plavix tablet supply chain was studied in two levels including the pharmacy and the hospital. The inventory variables were identified and mathematical relations were stated for feedback loops. Then, causal loop diagram (CLD) and stock and flow diagram (SFD) were designed. The simulation was performed for a 30-day interval. Results: The results of Plavix tablet supply chain simulation for a 30-day interval showed a remarkable increase in the demand and decrease in inventory levels in a way that 39% of the patients were not served while some extra tablets remained at the end of the study period. Conclusion: According to the cause and effect study of the simulation model, Plavix inventory problems were due to the increased demand in the studied time period and the inconsistency in the supply chain of this drug in Razavi Hospital. The proposed model of this study can help the policy makers of pharmacies and hospitals to achieve accurate prediction and coordinated decision making in ordering and safety stock holding policies.
