Introduction: Discharge of patients with personal satisfaction is one of the problems in treatment systems all over the world, which causes the recession and ultimately leads to jobs and poor financial health centers. This study, presents ranking of causes of discharge of patients with personal satisfaction in treatment centers in the city of Yazd.Methods: This study is a cross-sectional, descriptive analysis. The sample size is equal to 174 of 9896 patients, who have been discharged from hospitals with personal satisfaction. We also asked six expert's opinion. The method of Sampling from patients is stratified randomization and from experts is census. In order to data analyzing of patients, we have used the SPSS16 software and Chi-square Test, and for data analyzing of experts we have used Microsoft Excel 2010 and Fuzzy AHP Technique.Results: The most important causes of discharge patients in order of preference are as follows; financial difficulties and the cost of treating patients (36/2%), Sense of recovery (27/8%), Tired of hospital environment and Prolongation of treatment duration (18/4%), and from expert’s view, the most important cause is: those causes related to the crews and hospital staff.Conclusion: According to studies carried out in Iran, the rate of discharge is high compared to other countries mostly due to causes related to the patient. The clinical social workers, patient awareness of potential complications and make good and pleasant atmosphere for patient, in order to increase the level of personal satisfaction, should be considered as priorities. Administrators can also use the Fuzzy AHP technique to prioritize causes and then give the priority plan.

Introduction: Choosing the right type of device is important to support users of Health Information Systems. In addition to cost, durability and simplicity, staffing levels and hospital workflows affect in the selected. The purpose of this study was to identify the preferences of users in device selection.Methods: This applied research with qualitative method, Focus group, was conducted in 2014. Population research is users of HIS. They have been divided into three distinct groups based on their computer skills and medical knowledge. The sixteen users were invited to three focus group meetings and have been asked to explain their criteria for selection of the device. Questionnaires were also used in order to compare the results of discussions quantitatively. The reliability and validity of the questionnaire were confirmed by Cronbach's alpha (0.86). The analysis was performed using descriptive statistics.Results: The mean response of participants shows that end-users with high level of medical knowledge and computer skills, prefer to use tablets (3.75) and then PCs(3.67). Medical staff without computer skills selected tablets(4), workstation on wheels (WOW)(2.5), wall mounted workstation(2.5) and PCs (2.25). Ultimately members with skills in using the computer and lack of medical knowledge, selected PCs (3.37), tablets(3.29), Personal Digital Assistant (PDA) (3.17), wall mounted workstation(4.2) and WOW(2.67).Conclusion: This study showed that the best device for the medical staff is Tablet and PC is the best device for administrative staff.

Aim of this research is to study the Role of Information Technology tools in the relationship between knowledge Absorption capacity and organizational learning in Tehran Social Security HospitalsMethods: This research, in the base of method is descriptive-survey and in the base of goal is applied. To collect data on literature research studies such as library books and journals and to gather data for analysis of research hypotheses, the questionnaires were used that Its face validity was confirmed by professors and Its reliability (Using Cronbach's alpha coefficient: 0.92) was Confirmed. The population of the study was managers and supervisors of Tehran Social Security Hospitals that According to Cochran sampling formula and using quota sampling, sample size determination and 113 questionnaires were distributed among them. For analysis of statistical hypothesis testing, structural equation modeling was used.Results: The results indicate that the knowledge Absorption capacity has a positive and significant impact on organizational learning and information technology tools has a significant positive role in this relation.Conclusion: The Social Security hospitals promote organizational learning with incentives and ability of staff and improve and accelerate the process of organizational learning with Using updated information technology tools.

Introduction: Although the current architecture of information systems as necessary step in the process of establishing hospital information systems have been considered.Continuous evaluation of hospital information systems to determine the extent necessary to achieve its objectives. This study aimed to develop indicators to assess hospital information systems architecture has been conducted.Methods: The present study is a qualitative study using the Delphi technique in 2013 in Tabriz University of Medical Sciences has been done.Twenty expert’s in the field of information systems through purposive sampling and the availability of medical sciences universities were selected.The main method of data collection questionnaire based on Likert scale, the literature review was conducted to determine its constituent elements Interviews as well as additional methods of data collection were used. Validity through content validity and reliability of the test - retest was confirmed by experts. Data analysis was performed using SPSS software.Results: Developing indicators for the assessment of hospital information systems architecture, the six main components including functionality, reliability, being employed, efficiency, maintainability and portability consists of 86 indicators were extracted from the data analysis. For each of the main components of the 12 indicators, 8 indicators, 24 indicators, 21 indicators, 13 indicators and 8 indicators and methods of calculating each indicator was introduced.Conclusions: This study assessed indicators for hospital information systems architecture provides and hospitals can use these indicators to evaluate their hospital information system software.

Introduction: Content analysis applied for comparing texts, a writer’s works with others or the same author, the texts written in a natural language with texts in other languages literature and review of specialized field. The present study aims to analyze the articles to extract the sentences structure in order to support medical researchers.Methods: This study investigates 4-word lexical bundles in medical research articles. To this end, a corpus of 2,400,000 words was established based on 33 subject areas in medicine and frequency of 20 and distribution numbers of at least 5 were considered as criterion. Word smith tools 5, Antconc3.23, ABBYY Fine Reader to professional edition were used for the identification of lexical bundles. Then, identified lexical bundles were classified according to their functions and structures in the medical research articles.Results: Totally, 102 lexical bundles with different structures and functions were identified in medical articles. The findings of the current study showed that most of the medical writers use text-oriented bundles in their articles.Conclusion: This study presents new insights in better understanding of medical research articles related to specific area and further researches in written academic discourse and English for Academic Purposes. It also broadens new horizon on areas of text analysis and text statistical assessment.

Introduction: Human resource is one of the most important resources in an organization. role transparency can cause the usefulness of individual and finally leeds to doing things better.Considering the importance of librarians’ role transparency, the aim of this study was to determine the relationship between role transparency and Job satisfaction of librarians in state universities in city of Isfahan.Methods: The research method was descriptive-survey and using stratified random sampling, the sample was 105 persons among the all librarians (164) of state universities in city of Isfahan. The measuring instrument is questionnire. The validity of the questionnire was determined by content validity and the reliability by Cronbach Alpha and the reliability of the role transparency and job satisfication was estimated 0/89 and 0/91 respectively. In order to analyze the data, using SPSS software, Correlation coefficient, one-way ANOVA, Independent t-test and LSD post hoc test were used.Results: According to results, there is positive and significant Correlation between librarians’role transparency and Job satisfaction in libraries of University of Isfahan, Isfahan University of Technology and Isfahan University of Medical Sciences (p=/001).The findings also showed that between librarians’role transparency and Job satisfaction components including communications (p=/018), wages and salaries (p=.037), supervison (/003) and working conditions (/046), there is positive and significant relationship. But there is no significant relationship between librarians’role transparency and other Job satisfaction components.Conclusion: Role transparency causes job satisfaction and finally insight and attitude of individual toward his duties changes. Considering the importance of librarians’ job satisfaction and positive Correlation between librarians’ Job satisfaction and their role transparency, investigating ways to increase librarians’ role transparency was offered.

Introduction: Organizational Innovation requires to turns ideas into organizational usable forms and this idea are used to promote organizational performance. The aim of this study was to survey Organizational Innovation (Administrative & Technical) in the Academic Libraries of Shiraz public university (Shiraz University, Shiraz Medical Sciences University).Methods: This research is a descriptive – survey study, and the population consists all of managers and librarians in academic libraries of Shiraz in 2011-2012 years (151 persons), that by used the formula of sampling i.e. Cochran and were selected 86 persons. Sampling method is Random stratified sampling proportionate to size. To collect data a researcher made questionnaire was used which its face validity was approved by experts and its reliability was estimated by Cronbach's alpha coefficient (0.93). For analyzing the data SPSS V.16 software was used in two levels of descriptive and inferential statistics.Results: The status of Organizational Innovation and two component’s Administrative and technical innovation in the academic libraries of Shiraz Public Universities is more than the average level (3.42±0.66), but the difference between the components of organizational innovation (technical and administrative) were significant (P≥0.05). Also among the respondents’ views, there is no significant difference based on demographic variables of gender, field of study, years of service, and level of education (P>0.05), there is significant difference based on university and type of work in the library in the components of administrative innovations (P≥0.05). In other words, manager's greater administrative innovation than technical and public services staff.Conclusion: Academic libraries have been particular importance in this case in regard to increasing importance of innovation in organizations. Management academic libraries can improve their services and to better meet information needs clients with provide appropriate working environment for their employees and librarians.

Introduction: Awareness of Outlook, objectives, benefits and impact of telemedicine technology that can promote services quality, reduce costs, increase access to Specialized and subspecialty services. Therefore, the aim of this study was to determine the strengths, weaknesses, opportunities and threats in the telemedicine strategic planning from the managers and expert’s perspective in the health department, Isfahan University of Medical Sciences, in order to take a step towards facilitating strategic planning and approaching the equity aim in health in the province.Methods: This was a descriptive study, that data collection was done cross-sectional. The study population was composed of all managers and certified experts at the health department in Isfahan university of Medical Sciences. The sample size was 60 patients according to inclusion criteria. Information was collected by interview method. Researcher attempted to use the structured and specific questionnaire Then were investigated the viewpoints of experts and managers about determinative factors (strengths, weaknesses, opportunities and threats) in the strategic planning telemedicine. Data were analyzed using descriptive statistics (frequency, mean) and software SPSS 18.Results: Data analysis showed that change management (100%) and continuity of supply of credit (79/3%) were weakness point within the organization and strengths of the program were, identity and health telemedicine programs (100%), goals and aspirations of the current directors of the organization and its compliance with the goals of telemedicine (100%), human resources interested using computers in daily activities in peripheral levels (93/1%). Also organization in the field of IT professionals, had opportunities, and repayment specialist’s rights by insurance organizations is a threat for it.Conclusions: According to the strengths, weaknesses, opportunities and threats points determined by managers and experts, and compare it with success and failure factors, which are defined by different researchers, it seems will be fail to implement of telemedicine in the province at present. But according to the strengths identified by managers and experts, there are a lot of potential for telemedicine in the province, and may be used, in relation to telemedicine projects, with a 3 or 5 year strategic plan, and taking steps to get closer to the equity aim in health.

Introduction: The Quality health care issue that has attracted a lot of attention, Because Customers satisfaction is very effective in the health system. The Aims of this study was perceptions and expectations of the quality of health care in urban and rural Units health care providers.Methods: A descriptive and analytical study was conducted in 2014 in Qazvin province. The Statistical Society was all people in the province of Qazvin. Due to sample size estimation of the proportion of the population in 1002 were examined in the 6city of Qazvin and its villages. Data collection tool was a questionnaire SERVQUAL including 5 Scope: tangible, reliability, responsiveness, assurance and empathy based on the Likert scale. Cronbach's alpha value was 91 percent and test-retest coefficient of the study was 83 percent. Data entered to SPSS software and analyzed by paired Test and Pearson correlation. Results: The mean age of the population was, 32± 9/9 and 73 ± 47 minutes average waiting time for receive services. The most frequent recipients of services based of education, the education Less than high school diploma 3.43 percent (344 cases), followed by education diploma with the 29.6 percent (297 cases). Average perception in aspects of tangible (physical), reliability, responsiveness, assurance and empathy was 3.53, 3.54, 3.53, 3.70, and 3.39 respectively. Also expected was to be equal in size 4.58, 4.62, 4.58, 4.59 and 4.55 respectively. The maximum amount gap with the -1.155 was related to empathy scope and the reliability gap with the least amount of -0.888 related to ensuring scope and was statistically significant.Conclusion: According to the results of the study will be offered, Planning and policy for the promotion of empathy scope considered.

Introduction: The significance of hospitals as part of an impact on the healthcare system on anyone not wearing. In addition to providing quality service to enhance the likelihood of successful treatment creates a distinction between provider organizations and enhances are the competitiveness of the organizations. This study attempted to measure the quality Afshar Mohammad Sadiq of hospital care using Fuzzy TOPSIS and Fuzzy AHP.Methods: This is a cross-sectional analytical study was conducted in the second half 92 .The sample group of selected experts and patients in the Afshar Hospital Yazd in the fall. First, a literature review of research on service quality dimensions were identified from the hospital. After refining the dimensions, using a form of paired comparisons of experts (including the Chairman and teaching hospitals, nurses, technical staff of hospitals, health care management professors and faculty Medical Sciences and Health Services, Yazd martyr) surveys were performed and the coefficients of each of the dimensions of the analytic hierarchy process in fuzzy environment (the software Expert Choice) was calculated. Third, using questionnaires (reliability, validity and approved by relevant experts Cronbach's alphaof .92) based on a Likert - Elect of the patients admitted to hospital departments (cardiology, orthopedics, gynecology, and surgical) of 100 participants were randomly sampled using fuzzy TOPSIS (software TOPSIS) was ranked sectors mentioned.Results: Dimensions of responsiveness, assurance, security, tangibles, health communication and customer orientation as aspects affecting the quality of the hospitals were identified using the FAHP safety factor of greatest significance and importance had the lowest coefficient of tangibles (0.25406 in contrast, 0.88306 ). Also analyzed the data using FTOPSIS, cardiac and surgical wards of the highest and lowest ci (One option is to calculate the relative closeness to the ideal solution) achieved (33.457in contrast. 0.770).Conclusion: The results of this method showed that the highest priority to security experts, among other aspects affecting the quality of service. Also part of the heart disease, urology, gynecology and surgery are the first place to fourth in the provision of services.

Physicians’ Civil Responsibility has always been among important legal issues and thus, subject to rules faced with various challenges rooted in different approaches towards civil responsibilities. The present study investigated some of the world’s legal systems and different dimensions of responsibility to find proper strategies for securing patients’ rights as well as other issues in this respect. The study was a library research in which Iranian law and justice databases such as the Information Center of Judicial Institute and Iran's Legal Database along with sixty other resources have been exploited directly and indirectly within the period from May 2014 to September 2015. It has investigated legal and jurisprudential texts, internal and international rules and judicial processes acted upon in Iran, France and America in a comparative manner. Actually, the key points of the study turn round civil responsibility, fault, risk, irreparable loss, permissive waste, agreement and acquittal. The findings showed that Iranian legal system has not been able to take a fixed stance towards physicians’ civil responsibility in the past, e.g. in a certain period of time, based on some of the jurisprudents’ views, it has sometimes adopted risk and some other times fault as the basis for Physicians’ civil responsibility. However, in the course of time, it has come to follow a more optimistic approach which, if continued, can secure the patients’ and people’s rights in a more acceptable manner.

Introduction: All managers are responsible for planning, organizing, leading and controlling. Managers must have some competencies that enable them to function effectively. These competencies are often competency models. The aim of this study was survey of competency models in order to extract their essential competencies. Methods: This study was a systematic review that articles were searched in database "Google scholar"," Emerald", "Proquest", "Scopus" with key words "Competency & hospital management"، Competency & healthcare manager""، "Competency & healthcare administration"," Competency & health administration", and Competency & health executive". The results of selected studies were performed by 2 independent persons and based on qualitative targets were extracted and analyzed. And finally 12 articles that were contain questionnaire or model for hospital management competency, were selected.Results: Different competencies models were reported in 8 articles and reports. In order to compare these models, we decided to select 15 most frequent competencies in expand range of studies. Our observation identified that "communication skills" and "knowledge of health environment" were reported 7 times and selected as most important competencies. The Robbin Pillay was most completed among the models.Conclusion: In order to screen and estimate of hospital managers’ performances, we needed competencies index categories. Dependent on each countries condition, these models can be used as an appropriate method to determine an individual competency.

Introduction: The goal of lean thinking is value creation and the eradication of waste. The purpose of this paper was systematic study of items and rate of implementation the lean management tools and techniques in Iran’s healthcare system.Methods: In this systematic review that was conducted in December 2014, by searching keywords related to lean management in Iranmedex, Magiran, Irandoc, SID scientific databases and Google search engine, attempted to search the Research has been conducted in Iran’s healthcare centers regardless of the time of publication. A total of 10,550 Research was found. After reading them, 10478 Research were excluded from the study because of the irrelevant, repetitive and lack of access to the full text and abstract. Then attempted to classify and statistical analysis of 72 remaining researches via Excel 2010 software.Results: 5S(6.9 percent), Kaizen(8.3 percent), visual control(1.4 percent), value stream mapping and simulation(18.1 percent), value stream mapping (2.8 percent ), simulation(5.6 percent ), Poka-Yoke(9.7 percent), Hospital Information System(26.4 percent), Maintenance and Repair(5.6 percent) and identification the waste(15.3 percent), were identified as the most important lean off practices in Iran healthcare centers. 84.7 percent of the researches were published in 2007 onwards.Conclusion: The findings showed that the much of the tools related to Lean Management has been applied in Iran’s healthcare centers in the last eight years. The computers have played the greatest role in lean off. The interest of Iranian researchers has increased about lean healthcare. Deployment, collateralizing and propagation of lean healthcare with the help of industrial engineers are the main offers to policy makers in the healthcare sector.
