IntroductionA 65-year-old patient who was admitted for heart surgery was transferred to operation room again in a specialized hospital in Tehran so that the accumulated fluid around his heart and lungs can be emptied. Experiments and resampling showed that the first surgery had been mistakenly done on the patient; the patient who was supposed to undergo surgery at that time was the next bed and hospital personnel had taken the other one to operation room by mistake (1). In Iran there is no documented information on the rate of medical errors; however, incidents such as the above case represent the highest rate of medical errors due to lack of proper identification of patients. Approximately, 25% of hospitalized patients face some kind of medical error some part of which is due to improper identification of patients (2). Radio Frequency Identification (RFID) means identification through radio waves which is one of the important members of the family of Automatic Identification and Data Capture (AIDC) based on electromagnetic waves (3,4). From RFID system, data are automatically extracted using special tools, after which it is transmitted to users at the desired time and place. RFID has been recognized as one of ten top global techniques in 2004-2005 which simultaneously covers the areas of information management, recognition, and tracking, authentication, and process automation (4). If patients who need surgery have RFID wristbands with related information and digital images from patient, clinical team will be able to deal with patients correctly. Moreover, electronic interpolation ensures the team about following the right process, and if a patient is taken into the operation room mistakenly, the mismatch and an error will be identified automatically and immediately (5). In this regard, the results of a research done by Aguad and colleagues on "Benefits of Connecting RFID and Lean Principle in Health Care" (2005) showed that given the complexities and challenges of today's world, this technology can boost the performance of the medical sector and decrease human errors (6). Cangialos et al. (2007) represented their satisfaction regarding implementation of RFID in the studied hospitals in their study titled "Applying RFID to patient Care: Challenges and Opportunities". Their results show that this technology facilitates turnover in hospitals, decreases nurses and personnel's work load, and leads to a better interaction among physicians, nurses, and patients (7). Liu et al. showed that the use of RFID-based monitoring system can help in management of surgery process, patient positioning, identification, and follow up surgery care. Moreover, this system provides continuous monitoring of work processes and makes the information available to users in order to promote management efficiency. This system is a valuable management tool to address security, quality, and efficiency in surgery wards (8). NJ hospital in India is an example of successful implementation of RFID in monitoring security and management of pediatrics ward (9).Conclusion The main reason for some medical errors is inadequate access to information at decision-making time as well as poor communication among care team healthcare providers. This issue is critical in a place such as surgery room where access to on timely information on the patient's status is essential to ensure accurate performance of medical personnel. RFID technology can bring about efficiency and profitability of health care services through creating on timely information and its comprehensive understanding of healthcare personnel. Use of RFID technology will increase productivity and quality of services in hospitals and healthcare systems.

Introduction: Selecting an appropriate method for modeling and analyzing health data based on available data is very crucial. This study was conducted according to Probabilistic Neural Network (PNN) to detect if the coronary artery is closed or not.Methods: This study was diagnostic and it was implemented on patients of Kowsar Hospital in Shiraz, Iran who were exposed to Coronary artery angiography in September 2013. The number of population was calculated based on related formulation and the division of neurons in hidden layer by error rate of 0.1. Therefore, 152 patients were randomly selected for this research. In these implementations, 85% of data was used for training phase of network and 15% for the test phase. In this study, Probabilistic Neural Network (PNN) was used for prediction of coronary artery disease. The proposed neural network was implemented through facilities and functions of MATLAB software (7.12.0version) and simulated by a system of core i5, 2.4 GHz processor and 4GB memory and windows7 as operating system.Results: Performance indicators of this system were sensitivity and specificity. The presented system performance on the basis of these indicators was achieved1 and 0.94, respectively. Ultimately, the designed and implemented system could confirm its superiority for diagnosis of patients of coronary artery according to similar studies.Conclusion: The results of this research indicated that in the studied population, probabilistic neural networks could achieve more accurate diagnosis for coronary heart disease comparing other studied neural networks. Due to high specificity and sensitivity of the system, it can prevent the possible side effects and injuries of angiography for the patients who don’t need it. And also, it can distinguish the patients who really need diagnostic actions in the least time and the most accuracy.

Introduction: Clinical governance is a key tool for managing and monitoring of information. However, use of information is one of the fundamental components; but unfortunately, it does not pay much attention to information management. The purpose of this study was investigating situation and presenting conceptual model for information management based on clinical governance.Methods: A cross sectional study was conducted in 1392. Population of the study consisted of all clinical governance units of the Kashan University of medical sciences affiliated hospitals and sampling wasn’t conducted. Data were gathered through questionnaires and interviews. Reliability was assessed in the test -retest. Face and content validity of the questionnaire has been confirmed by experts. Data were collected from a pilot hospital and reforms were carried out and Final questionnaire was prepared. Data were analyzed using descriptive statistics and SPSS 16 software. With the scenario derived from questionnaires, respectively entity relation Diagram (ERD) and Unified Modeling Language (UML) are presented by using Visio 2007 and Rational Rose 7 software.Results: The results showed that 25/79 percent Indicators of the hospitals were calculated. Database was not designed (zero percent) in Clinical governance units of hospitals. 3/83 percent of the hospital's clinical governance unit has announced the need for a databaseConclusion: Clinical governance unit of hospitals to perform its mission, do not have access to all the needed indicators. Defining of Processes and drawing of models and creating of database are essential for designing of information systems.

Introduction: The radiology management information systems (RMIS) are of particular importance in facilitating managers’ decision-making, controlling and planning roles. This research, hence, aimed to study the RMIS in hospitals affiliated to Tehran and Shahid Beheshti University of Medical Sciences.Methods: This research was a descriptive and cross-sectional study. To conduct the study, the hospitals affiliated to Tehran and Shahid Beheshti University of Medical Sciences was classified into two groups, and random sampling was used to select hospitals from each group. In total, the sample of the study was composed of 21 hospitals. Data collection was conducted using observation and interview using a questionnaire and a checklist. The Validity of research tools was reassured using the method of content validity. The Reliability of questionnaire was checked using test-re-test method (r=0.89), and data were then analyzed using descriptive statistics.Results: The findings showed that a mixture of paper-based and computer-based methods was used in the majority of radiology management information systems (RMIS). The data related to equipment, were gathered with 27.8%- 44.4% and this was 16.6% -33.3% for administrative and financial data. The main sources used for data acquisition were hospital information system (HIS) in computer-based systems, master patient index (MPI) and patients’ insurance documents in paper-based systems. Among the reports prepared, financial reports and those for key indices had the lowest frequency. In all centers under study, the RMIS was integrated with hospital information systems, and integration with other systems reported as 16.7%-27.8%.Conclusion: Development and improvement of RMIS would be facilitated considering complete gathering of data related to personnel, equipment and financial affairs; accurate and complete data processing as well as appropriate distribution of information.

Introduction: Medical Records services as one of the indicators of Allied Health Sciences in order to demonstrate the work efficiency and quality of healthcare work is a valuable benchmark. Medical records department play an important role in assessment and planning of prioritization quantity and quality of healthcare and its staff from legal and ethical perspective should be held accountable for the quality of information registry and this fact have direct connection with services quality so their perspective in determining the quality of services in health institutions has a special position. This study evaluates the quality of medical records department services from the perspectives of medical records personnel in teaching hospitals of Isfahan.Methods: This was an applied and descriptive- survey research with statistical population of medical records personnel employed in teaching hospitals of Isfahan in 2013 (96 people, all the research population). In order to collect data Servqual standard questionnaire based on the five dimensions of assurance, responsiveness, empathy, reliability and tangibles were used based on likert scale rating. Since in preparing the questionnaire, qualitative perspective of Parasurman - Zeithamel was observed, and with attention to consulting with experts, questionnaire’ validity was confirmed. The reliability of the questionnaire calculated by Cronbach's alpha coefficients and was obtained 0.83 for the expectation and 0.88 for perceptions. SPSS software for data analysis and descriptive and inferential statistics were used.Results: Two-way analysis showing the relationship among the gap between expectations and perceptions of the medical records department personnel in services' quality. The results show a significant difference between expectations and perceptions of medical records departments’ personnel in teaching hospital of service quality. The gap between expectations and perceptions were equal to: assurance (0.98), responsiveness (1.82), empathy (1.94), reliability (0.60) and tangibles (1.53). The biggest gap was related to empathy (1.94) and the smallest gap was related to reliability (0.60).Conclusions: In general, the largest quality gap in this research is the empathy dimension of staff. Efforts and planning of hospital management and heads of medical records departments to create the circumstances in which number of personnel to be determined by using the standard method, based on the sheer size and number of patients; can lead to raising the quality of the services.

INTRODUCTION: Due to dynamic of terms, their classification is challenging. The current research aims at determining the usability of a model for automatic recognition of MeSH terms categories through measuring their occurrence frequency within relevant and non-relevant document corpuses from PubMed. METHODs: This is a descriptive research that uses the document analysis method. MeSH and PubMed were used to collect research data. The significancy of these resources confirms their validity. 18164 MeSH-term and 163226 PubMed documents were selected. The both of these amounts are greater than what Cocran function suggests. Eleven document corpuses were retrieved from PubMed. The relative ocurrence frequencies of MeSH terms within each corpus were determined. The results were compared with the real category of MeSH. In additions, the categories of 1 percent of MeSH terms were determined by experts in medical domains. The frequency distribution method was used for statistical description of data. Data were also analyzed through T and Chi-Squar tests in SPSS.RESULTS: Each document of PubMed on average belongs to three MeSH categoris and most of Mesh terms occurred in all corpuses. The results confirm that the suggested method increases the probability of MeSH category recognition. The performance of the method depends on the subject category of MeSH Term and ranges between 3 to 67 percent. The findings also show that the medical expertises determination on the subject category of MeSH Terms is compatible with the real categories of MeSH tree. CONCLUSION: The compatibility of the subjective and objective methods for the subject category recognition depends on the knowledge area. The subjective categorization is a quite cognitive task and roots in human environmental experiences. This is why the machine depended models are not able to simulate that process.

Introduction: An ideal society needs to provide health with high quality life and to profit from a healthy environment. To achieve this goal, the health system of the country has compiled a comprehensive scientific health plan. Doing research on investigating study fields in nursing is limited while such studies could be a great step to improve country’s health system. The present study aims at investigating degree of conformity of previous researches in nursing system with country’s comprehensive scientific health plan.Methods: In this descriptive cross sectional study dissertations of MS and PHD degrees were investigated. The study samples were 135 PHD dissertations that were included in the study via counting. The study tool was a checklist to assess dissertations. This checklist was prepared through studying relevant texts. Content validity was determined by Faculty of Nursing. The data was analyzed using SPSS VER. 18 and descriptive statistics (frequency and percentage).Result: Comparing the findings with the comprehensive health plan showed that most of the nursing researches were in care giving field. Besides, the number of studies in the fields of society and person-family were equivalent. In the field of prior studies, the majority of researches were on basic level while the minority was on developmental level.Conclusion: Investigating previous researches in medical universities and their conformity to elements of the comprehensive health plan could be an index to evaluate development of the plan; besides it could be considered as a strategy to conduct previous researches in medical universities.

Introduction: The Primary Goal of Physicians are salvation of the human life. Physicians can research through medical databases in causes of death, are closer to this goal. This study was conducted with aim to determine the amount of overlap causes of death in developing countries in PubMed and Scopus databases.Methods: Survey methodology, with sciencometrics approach and study has been applied. The research population consisted the 20 First results retrieved in the10 primary causes of death in the developing world In January in 2012 at the PubMed and Scopus databases. In order, The Mortality Factors was taken from World Health Organization (WHO), then to selection descriptors was used from Medical Subject Heading (MESH), then were searched in PubMed and Scopus databases. After Search the amount of total retrieved articles was distinguished, and The 20 first results in each factor with directly observation in 2 databases for subscription between them were investigated. using Excel 2007 version of the descriptive statistics (frequency) examined the degree of overlap was calculated by the formula. Results: Results of research showed that total retrieved articles in 9 Mortality Factors in Scopus was more than PubMed. Overlap only in 2 factors «Cerebrovascular Disorders» and «Dysentery» was 70% and 65%. In «HIV», «Tuberculosis» and «Diabetes Mellitus» two databases had not any Overlap.Conclusion: Results showed that researchers with need more articles and the Recall was important for them, it is better than used Scopus database. In order to amount of Overlap at two databases in various Mortality Factors were different and with regard to Organization Policies and Limitation Researcher and physicians can be use each two databases.

Introduction: Utilization of resources of evidence based medicine depends on quick and easy access to reliable databases, which meet different functionalities of a user interface, usability and technical issues. Given the importance of evidence based medicine resources in accelerating response to clinical questions, the present study is an attempt to evaluate compliance of evidence based medicine resources based on Ovid Criteria. Methods: This study is an applied work, which was carried through surveys and webometric methods between 2012 and 2013. The population was consisted of six evidence based medicine resources. Evaluations were done using Ovid criteria. The collected data was analyzed in SPSS (20) using descriptive statistics (frequency and frequency percentage).Results: Findings indicated that resources of evidence based medicine obtained less than half of the total rating Ovid Criterion. In general, they were placed at 36.92% level and obtained 319 points out of 864. Therefore; compliance of all resources of evidence based medicine based on Ovid criteria was at the low level.Conclusion: Despite the fact that resources of evidence based medicine are aimed to provide rapid access to medicine evidence; the problems regarding satisfactory ease of use, effective user interface and observance of the technical issues were undeniable. Dealing with these problems successfully, leads to quick and easy access to the evidences by users. It is suggested, based on the findings, those experts of database design and public information need to pay more attention to the components such as search, service, display, links, guides or documentation.

Introduction: payment systems can be an important tool in health policy. We evaluate the impact of different methods of payment (capitation, salary, fee for service payment) on the performance indicators of family physicians. The study aim is to identification and making priorities of family physician performance indicators to select an appropriate payment mechanism.Methods: this study used mixed methodology. There, several and different tools are used to gather data at any stage. Literature review has been done to identify Family physician performance indicators, in addition to literature review, expert panel also used and to prioritize performance indicator, payment mechanisms based on performance indicators and to choose best payment, AHP technique has been used. The data were analyzed by Excel 2007 and Expert choice 11 software's.Results: Of the 63 family physician performance indicators, the expert panel members achieved positive consensus on 10 performance indicator. With inconsistent of 0.08, Cost, preventive care and health promotion rate, patient satisfaction, physician satisfaction, referral rate, quantity, physician retention in underserviced area have been choose to be the most key performance indicators. in prioritization of payment mechanisms due to performance indicators, capitation method with inconsistency ratio of 0.04 was selected to be the best model.

Introduction: Patient safety is a critical component of health care quality. In addition to human suffering, unsafe health care exerts a heavy economic cost to the healthcare system. The aim of this study was to assess the Patient safety infrastructures in selected hospitals of Isfahan University of Medical Sciences.Methods: The present descriptive-analytic study was carried out cross-sectional in spring of 2013. Data were collected Using compulsory standards checklist that is used by World Health Organization (WHO) to assess patient safety in hospitals through observation, interviews and reviewing documents in selected hospitals of Isfahan . the data using SPSS 21 , descriptive and inferential statistical methods was analyzed .Then, based on the Compliance level with infrastructures of patient safety, hospitals were classified into three levels of poor (lower than 50%), medium (5o-75%), and good (higher than75%).Results: The overall compliance level of patient safety infrastructure mean with 55,3% was evaluated intermediate in studied hospital. The leadership and management with the highest mean (69.5%) was evaluated average and the patient and public involvement domain with the lowest mean (37.5%) was evaluated weak .Conclusion: In total, the overall compliance level of patient safety infrastructure was evaluated intermediate in studied hospital. Given that acquiring hundred percent of critical standards in in the four domains are essential to be considered at the basic level of patient safety friendly hospital. Therefore, Necessary investments in strategic and operational planning must be considered in order to achieve 100% standards in the four domains

Introduction: Human errors in medical area have made a concern amongst health policymakers and scientists. So, this study aimed to evaluate influential human factors on risk management in pre- surgery of Obstetrics and Gynecology and assessed the influential strategies and contexes on risk management. Methods: The research methodology is survey method and it is a type of applied research and the nursesو head nurses, health workers ،Doctors and all health staff associated with patient in Al Zahra health and education center of Rasht in 2013 who considered as statistical population of this research. 132 people was selected as the sample from the total population of the research in accidental method and the questionnaire that was designed by researcher, its validity has evaluated by reliability of 86/6was measured and the comments masters were concerned . Software spss version 21 Plus Descriptive statistics including mean and standard deviation of measurement techniques and test statistic t and chisqure investigated.Results: There is a significant difference between the human factors on risk management, and influential strategies and contexes on it. The results showed that the influential human factors affected on risk management were lack of attention to the most important medical history and diagnosis، inadequate health issues of non-compliance by the staff and services of patient transfer to surgery unprepared the surgical team for acceptance patient for surgery. Since the use of the Clinical and patient guides lines with regard to the full report, allocated sufficient funds and ،short-term and long term education risk have evaluated as the influential strategies and contexes on risk management.Conclusion: Since the Alzahra health and education center of Rasht is the biggest Obstetrics and Gynecology hospital of GUMS, it is necessary to survey influential factors on risk management. So, base on the results, human factors can cause possible risks in pre surgery in this center. Also, it is necessary for policymakers and managers pay attention to strategies and contexes that can reduce the rate of risks and can manage them.

Introduction: The goal of health system reform in Iran is embarking on a series of continued changes for improving efficiency, fixing justice for people's use of health services, protecting people against financing dangers due to disease, and reforming payment system. Family physician program is selected as the base of health system reform. The aim of this study is to determining the causes of discontinuation of family physicians working in Mashhad University of medical sciencesMethods: In this cross-sectional study 156 of family physician working in Mashhad University of medical sciences randomly selected and self-constructed Questionnaire completed for them. The statistical analysis was performed by using the Statistical Package for Social Sciences [SPSS version19].Results: More than half of family physicians (59%) were female. about 80% of family physician ages being between 25 to 42 years. Mean age of family physician was 32.7 ± 6.06 years. About 82% of family physicians were married. The average work experience in family physicians was 16 months. The main reason of discontinuation of family physicians was; low salary (26.7%), effect of monitoring score on the salary (11.7%), and high responsibilities of the physician (9/9%).Conclusions: factor related to the salary and effect of monitoring score on the salary with more than 38% was the most important reason of discontinuation in family physicians. According result of this study the ministry of health and medical education must create dramatic changes in the physician payment.

Information therapy including easy, available and low cost treatment methods that nowadays in most developed countries can be used as one of the important therapeutic approaches alongside other therapies. Unfortunately, in Iran the role of such medical librarians in it remains unknown. Information therapy is providing the right information at the right time for the right patient to improve people's mental and physical health and help patients. This study is a narrative overview and its aim is review of the definitions and concepts of information therapy, its role in educating patients, its advantages and the role of the medical librarians in that. For gathering appropriate articles, the key words "information therapy", "bibliotherapy", "medical librarians", "libraries " and "chronic diseases" are searched in databases SID, Magiran, ISI, Web of Science, Pubmed, Science Direct, Ebsco, Springer, IranMedex, Irandoc, Noormags, Elsevier, MDConsult, Emerald, and Proquest from Azar 1390 till Mordad 1392. After studying the retrieved documents, 40 documents were selected and reviewed. Literature review showed that medical librarians and information communicators have an effective role in improving health care processes through information therapy and its sub along with health care team including physicians, nutritionists, psychologists, counselors and specialists in other discipline. Meanwhile because novelty of such approach in Iran It is essential that the role of the medical librarian introduce and explain for related medical experts and officials, and the necessary infrastructure be done.
