Introduction: Perspective of Electronic Health Records (EHR) is not constant in different societies and facing with the opportunities and challenges. In this study attempted to identify challenges of EHR implementation of point of view of custodians in Isfahan Province. Methods: this research was a qualitative study that conducted in 2011 in Isfahan. The method of phenomenology was used so people's experiences through semi-structured depth interview about electronic health records as a new subject in the field of health has been studied. The study population included physicians(n=3), administrators(n=4) and experts in different fields(n=8) with at least a master's degree is related to the content and structure of electronic health records and inform research activities in this area have been or in the planning and introduction of electronic health records in the role of have contributed. Information obtained through hospitals and other relevant authorities in this field such as Deputy Treatment, insurance agencies, universities and General Staff were collected. Using the snowball sampling method that have first inclusion criteria were interviewed, because they then other people who possess the qualifications for entering the study were identified and referred to them for the interview was done. This continuum in order to reach saturation phase information (Information Saturation) and thus continued to study sample consisted of 15 people three doctors, four people from different parts of health managers and eight were experts in the field of electronic health records reached. Result: The researcher divided the challenges to the two areas of infrastructure and structural. Conclusion: Unfortunately, the results of this study showed although physicians are the most important users of electronic health records but at least the comments have presented. If the evidence is due to weakness of their role in the introduction and implementation of electronic health records, these disastrous results will follow. Introduction: Appropriate information is essential to plan and to implement clinical governance. This question is posed that what is hospital staff’ expectations of information. This study was conducted to evaluate hospital staff’s expectations of information in clinical governance. Methods: This descriptive - analytical study was performed on 258 practitioners of affiliated hospitals of Semnan University of medical sciences and social security organization in 2012. Data collection instrument was a valid and reliable questionnaire which was confirmed by experts and was measured its reliability by Cronbach's alpha coefficient. Then it was distributed among the population. Descriptive statistics were used for data analysis in SPSS 16. Results: The findings showed 71.7% of staff had attended training course of clinical governance. 58.6% chose clinical information as a first priority and 53.3% selected managerial information as the fourth priority in clinical governance. 3.6% chose information technology and informatics center as a first priority in the implementation of clinical governance. Conclusion: The results showed although most of the population had passed clinical governance course but it seems essential contents have not been provided in relation to the role of information in clinical governance in the course. These results can create an opportunity for experts in information technology and health information managers to show, more than ever, the role of information departments in the improvement of clinical governance. Introduction: research is main platform to deep and comprehensive understanding of facts and thought. Dissertations are as research résumé for universities and research centers, which some factors affect on their qualities. In the research detection of status of oroomiye University of medical sciences dissertations. Methods: the research done with survey and analytic method. 315 dissertation, select in base of Morgan table as sample among 1729 dissertation, with various degree and majors by stratified random sampling method that assess with checklist made by the researchers for validity. Data import to SPSS19 software and analysis by descriptive statistics and Chi-square formula. Results: the dissertations have serious defects in appearance factors for example in abstract, citations, page rank and etc. 69.5 percent of the dissertations used survey methods and 22 percent used experimental methods and 8.5 percent used review methods. Also 77 percent of dissertations are clinical and the remaining is nonclinical. Variables such as time spans (1365-1374 and 1375-1384 and 1385-1390), majors, degrees and sex affect on the research methods. Conclusion: some variables affect on universities dissertations quality. Supervision by dissertation responsible process, from subject selecting stage until final delivery, to controlling effected variables is necessary. Also in base of values of significant between methodologies of the dissertations V.S fields and degrees of graduates; it is necessary to making some guides and handbooks to driving the dissertation subjects and results to the society needs. Introduction: Today, knowledge management in libraries and information centers has found effective use that led to changes in their form and function. Whatever libraries use of knowledge management processes it provides favorable results in delivering information services.The objective of this study was to determine the establishment of Knowledge Management in the libraries of Isfahan University of Medical Sciences.Method: This survey was an operational study. Population was all librarians (82 person) employed in Isfahan University of Medical Sciences. Data collection was performed with the Pasture questionnaire. Alpha Cronbach was 0/97. Collected data analyzed by SPSS 18in two levels of descriptive and inferential statistics (T-test, F- test).Results: findings show policy and strategy, systems leadership, encouragement and reward systems, education systems and knowledge creation, knowledge management systems, training and communication systemThe results showed that all six components of the knowledge management in MUI Libraries were below average. Conclusion: According to research findings, establishment of KM elements such as: lack of guidelines and policies and statutory authority supporting is in non-academic way in MUI libraries. Introduction: Measuring the service quality in organizations is the necessary background for planning to improve the quality of their services. Quality services in the medical libraries has the fundamental role in achieving to community goals in the field of public health.The present study is done with the aim of gap analysis and ranking of service quality dimensions and also ranking of Medical Science Libraries of Shahid Sadooghi University.Method: This research was a survey study and its population was included 3339 people of Shahid Sadooghi University of medical science library users in the second half of 1391, that the number of 379 users have been selected by means of sampling method. Sample size was determined by Morgan table. Data were collected by libqual questionnaire; also 10 questionnaires that each included four “matrix of paired comparisons” was distributed among 10 library specialists. Formal validity of questionnaire was confirmed by thenumber ofnational and international professors and experts .the reliability of “ libqual” questionnaire and "Matrix of paired comparisons” questionnaire was confirmed by calculating “Cronbach’s alpha coefficient” for each dimensions of library service above 70 percentand “ rate incompatibility” less than 1/., respectively. Analysis of data obtained from libqual questionnaire is done by means of spss software in both descriptive statistics (percent of average and standard deviation) and inferential statistics (Friedman test); also analysis of data obtained from "Matrix of paired comparisons" is done by means of expert choice software. Results: data obtained from the “Libqual questionnaire” indicated that the libraries in all aspects related to "impact service" dimension can meet the minimum expectations of users that show their librarians have been modestly successful from the human dimension of service performance, while in some aspects related to dimensions “information access " and " library as a place" cannot meet minimum expectations of users. While none of the libraries surveyed cannot provide the ideal service for users.Also data obtained from the “matrix of paired comparisons" indicated that according to the experts, the “impact service” dimension and medicine – pharmacology library had the topmost rank among the service quality dimensions and libraries, respectively. Conclusion: as the library and information science experts believe that the impact service dimension is the most important dimension in the relation of improving the quality of library services, medical librarians indicated that they have been modestly successful from the point of user servicing and can meet the minimum expectations of users in comparison with the other dimensions.Keywords: Library Services; Service Quality; Libraries, Medical; Analytical Hierarchy

Introduction: Based on the fact that knowledge is not limited and scientific Exchanges help researchers to pursuit of new findings, this study aimed to determine citation rates of medical articles and books by faculty members of schools of Pharmacy in type 1 Universities of Medical Sciences in Iran, in text books at Amazon website, during 2000 to 2011. Method: This study was done using Citation analysis, a method performed by Scientometrics techniques. The study Population included 372 Text books indexed on Amazon website that have been published between 2000 to 2011.These books reference have been studied due to the citation to the scientific production of faculty members of school of pharmacy of Iranian universities type 1. Google Book search, a part of the Google search engine, was used as the data collection tool. Data analysis was done using (SPSS), version 18 Results: Citations to medical articles by schools of Pharmacy in type 1 Universities of Medical Sciences during the study period had an ascending trend, in which Tehran school of pharmacy (37.19) percent and Ahvaz school of pharmacy (1.03) percent had the highest and lowest growth, respectively. Most cited articles were original papers (89.6) percent and case reports ranked last with zero percent.Most cited articles were written by three authors (89.96) percent. Conclusion: The findings suggest that cooperation is the factor influencing to level of quality scientific papers and citations at the international level.Keywords: Analysis; Citation; Reference Books; Schools, Pharmacy.

Introduction:The most important goals of the health and cure services are promotion of life quality and equal access of all the people to aforementioned services. Higher health care costs and its financial pressures upon households' shoulders is one of the barriers of equal access to these services that policy-makers and academics have concerned about these rising costs.Methods and material: Using Heckman 2 stages model this research attempts to find out the effects of the factors cause to the out of pocket payment. The data is collected through a questionnaire.To do so, a sample of 312 patients admitted to hospitals in the city of Tabriz has been surveyed. Results:The results indicate that, income, age, being in private hospital, ruraland having an underlying diseasehave positive impact on the out of pocket cost. Also it found that, being in a private hospitals and having an underlying disease significantly have positive effect (due to greater than 2 t-statistic), and being a local patient and also type of admission, have significant negative impact on out of pocketConclusion:Income, age, being a private hospital, rural and has an underlying disease has positive impact on the out of pocket cost. So the government extending the insurance coverage for People and do the appropriate action forReduce direct payments out of pocket in the elderly and non-native Persons.Key words: Costs; Patient Discharge;Patients; Insurance

Introduction: In the health map of Iran in the fifth development plan, the increasing in productivity of main factors (capital, energy, raw material and labor) has been emphasized. This research, based on performance of provinces in Hygiene and health sector, investigated the role of macro variables of health in efficiency of provinces in the rural sector. Methods: This study has been done with descriptive- analytical method, among all provinces of Iran, and between the years 2000- 2007. Statistics are derived from official documents and in this regard, three input and five output indicators, mathematical model of data envelopment analysis and Diagnostic analysis are used. The validity of study was approved by the expert. Also, two software, DEA master1 and SPSS were used in this research. Results: This study showed that there are 9 efficient provinces, Tehran and Mazandaran are in the top (with efficiency figures of 295 and 174), and Kurdistan and Sistan are in the bottom of ranking list (with efficiency figure of 67 and 60). With using Sensitivity analysis and multiple diagnostic analyses, the most effective indicator was "Average per capita number of home health and rural health centers"(with 24 times sensitivity and effect coefficient of 0/5). Conclusion: Overall, it was determined that in the field of management of rural health and in the current conditions of the country, the importance of input indicators or the public health infrastructure are more than other factors. It also determined that in output indicators, performance indicators are currently more important than the health indicators. The particular innovation of this article is using "The average health and hygiene's cost of n rural household, provided by government and free of charge" indicator. Of course, about the importance and impact of this indicator, there are major differences between two methods, which are used. Keywords: Efficiency; Rural Health; Data Envelopment Analysis

Introduction: the establishment of each urban element in special spatial-physical position of the urban level is subject to specific principles, rules and mechanisms, in that if observed, it will be led to the functional success and effectiveness of that element in that specific location.The main goal of this research is to investigate how the health-therapeutic centers in Nourabad city are spatially distributed and the positioning of health-therapeutic centers in Nourabad of Fars province. Methods: the research method in this study is a combination of documentary and descriptive-analytical methods which the appropriate locationshas been specified for building new health-therapeutic centers as well as the best and nearest route to the hospital using GIS software and index overlay model, Analytical Hierarchy Process (AHP) and network analysis model in Nourabad in 2013. Results: studying the health-therapeutic centers in Nourabad city shows that this center is in an unsatisfactory condition in terms of the number and distance of availability and the best locations for the construction of health-therapeutic centers was determined using the criteria of slope, population density, distance from existing health-therapeutic centers, green space, firefighter, communication network, the type of land use and finally the incorporation of these information layers. Conclusion: after reviewing suitable sites for the construction of health-therapeutic centers and its adjustment with ground realities, multiple locations for constructing hospitals and new health centers for this city have been suggested. In addition, the best and nearest route for the health centers and hospitals in different parts of the city for faster and better access of the citizens to these centers was determined using network analysis in GIS software. Also, the results of this study show the effectiveness of Geographic Positioning System (GIS) in urban planning, especially in locating health-therapeutic centers. Introduction: Pharmacies as the only official reference of delivering medicine to patients play an important role in medication and counseling medication to patients. In this study, we investigated the status of Satisfaction of service quality of Bushehr pharmacies using the Kano and SERVQUAL models. We also presented executive solutions for improving the current status of services. Methods: This Study is practical in terms of objectives, and also descriptive. Our sample contains customers and visitors to the pharmacies of Bushehr. Our questionnaire we used random sampling for determining the number of our sample.Results: Our results showed significant differences between the quality of services provided by the pharmacies and the perceptions of the Customers. From 29 features, 17 features have negative gap. We also proved that there is a significant gap between prices of medicines and medical supplies, etc., location and distribution of pharmacies convenient boarding, compensation in case of failures in customer service, the service time (medicine delivery) and the suitable number of staff to provide services to Customers on time.Conclusion: There is a strong and positive correlation between indicators of quality pharmacy services and satisfactory of Customers. Therefore the satisfaction of patients can be increased with improving the quality of services. Keywords: Satisfaction; Pharmacy; Service Quality; Clients.

Introduction: today, social capital is a need in society. Also, leisure time and physical activities are among the most important productive sources of social capital which has noticed nowadays. This survey studies the relationship between social capital and physical leisure time of faculty members of Esfahan University of Medical Sciences.Methodology: descriptive correlation method is used in this study. Two questionnaires are used for data collection. Social capital questionnaire is based on SCOT Model. Also, leisure time questionnaire made by researcher which Face and content validity of it was verified by experts. Reliability coefficients by using Cornbrash’s alpha coefficients were calculated at 0.92 and 0.82 respectively. Sample populations measured by Khan’s Formula, they are 150 people that were selected as sample using multiple cluster sampling by taking the sex and college into consideration as variables.Findings: according to the findings there is a direct relationship between a combination of social capital parameters (include commitment, attitude, trust, participation, mutual relationship, social norm and unity) with the way of spending physical leisure time. (R = 0.0659, P = 0.001). From among the parameters commitment is the only parameter which is significant with Beta coefficient(B = 0.293, P = 0.044) and social norms with Beta coefficient (B = 0.196, P =0.047), but the rest are not significant. Conclusion:Playing sport and doing physical activities in the leisure time and also taking part in group activities and their membership provide a situation for people to respect the group interests through communication. Such activities can cause the level of social capital and it`s factors to be increased.Key words: Social Capital; Exertion; Leisure Activities.

Introduction:Patient satisfaction is one of the most important indicators of quality of service in hospitals. Pay attention to the views of patients,is the cause of their satisfaction and a foreground for improving the quality level of hospital services. The present study has been to assess patient satisfaction after executing autonomy plan with the goal of improving the quality of service.Methods: Using a cross-sectional descriptive study, 109 questionnaires based on SERVQUAL model and 5 ranking scale of Lickert were distributed among of admitted patients in Amir- Al Momenin (PBUH) and Saheb Al-Zaman (PBUH) hospitals both of before and after executing autonomy plan in Shahreza, Isfahan in March-april, 2012. The obtained data were analyzed in descriptive level (frequency, percentage, mean, and standard deviation) and inferential level(Pearson correlation test, independent t-test and ANOVA test)by software SPSS version 15.Results:Based on the results of this study, patients satisfaction, very much in after autonomy than before, is the evidence of improving the quality of services after executing autonomy plan so that satisfaction of "very high", in percent more patients after autonomy(34/7) compared to before autonomy (11/7) from quality hospital services is seen.Conclusion:According to the results,the hospital should pay attention so much to the quality of service,in tangibles diameter that related to face and physical aspect, and fast responsiveness to customer service, especially in some parts that are front-line of hospital services.Keywords: Patient Satisfaction; Evaluation; Hospitals; Quality of Health Care

Bibliotherapy is one of the complementary methods of treatment to cure those suffering from mental or behavioral problems. This research attempted to answer the question:"Are present librarians competent to do bibliotherapy?" This study is a review type; the data were gathered through reviewing relevant Persian and English sources. All Persian articles published as the result of local research were also studied. In this study, the literature on necessary traits and skills needed by a successful therapist as well as the curricula for medical librarians and Knowledge & Information Science students were also reviewed and analyzed.The present curricula are deprived of any courses dealing with bibliotherapy. Further, the present librarians do not enjoy required competencies as a successful therapist. Thus, librarians are not qualified to do bibliotherapy independently.Due to the importance of bibliotherapy as a treatment method, it is essential that librarians refrain from involving in bibliotherapy independently. They may use it when assisted by specialists in psychology, counselling, and/or psychiatry. Due to relative overlap of Knowledge & Information Science and Medical Librarianship fields with bibliotherapy, and highly motivated librarians, library departments may establish bibliotherapy courses with the help of medical experts.Keywords: Bibliotherapy; Librarians; Complementary Therapies.

Introduction: Evaluation of the effects of health information systems enable health care policy and decisions makers to better invest on and utilize health information applications. The objective of this systematic review is to consolidate existing evidence from published studies on the effect of health information systems in Iran. Methods: The aim of this study is systematic review of articles about effects of health information systems in Iran. In this study all papers relevant to the research objective were retrieved from SID, Magira, Irandoc, Medlib and Iran Medex databases until July 2013, and finally 18 out of 861 retrieved articles were identified as relevant and reviewed by the authors.Results:This study shows that approximately more than half of the studies indicated the positive effects of these systems on costs and less than half of them had negative on costs.The results also demonstrate other major effects of these systems: decreased medication errors, increased accuracy and timeliness of activities, increased accessibility of information and improved quality of services.Conclusion:Theresults of this study provide a good insights concerning the effects of health information systems for policymakers and researchers. This study mainly highlighted the positive effects of these systems on health care proceduresKey Words: Health Information Systems; Hospital Information Systems; Quality Improvement.
