چالش‌های ناشی از تقاضای القایی خدمات سلامت؛ یک مطالعه‌ی کیفی

چکیده
مقدمه: تقاضای القایی یکی از چالش‌های نظام سلامت کشورها است که باعث بالا رفتن سهم مردم در هزینه‌های درمانی و همچنین رشد شاخص هزینه‌های تحمل ناپذیر می‌شود. در نتیجه مقاله‌ی حاضر به دنبال شناسایی چالش‌های تقاضای القایی با استفاده از تجارب صاحب‌نظران دانشگاه علوم پزشکی اصفهان بود. روش بررسی: روش تحقیق که در شهر اصفهان در سال 1391 انجام شد، به شیوه‌ی کیفی بود و از مصاحبه‌ی نیمه‌ ساختار یافته برای گردآوری داده‌ها استفاده شد. برای تعیین روایی و پایایی معیارهایی مانند موثق بودن اطلاعات، قابل اعتماد بودن، قابل اطمینان بودن و قابل تثبیت بودن داده‌ها درنظر گرفته شد. جامعه‌ی پژوهش شامل صاحب‌نظران دانشگاه علوم پزشکی اصفهان بود. نمونه‌گیری به روش هدفمند انجام شد و تعداد 17 نفر مورد مصاحبه قرار گرفتند. به مصاحبه‌شوندگان اطمینان داده شد که نامی از آنها برده نمی‌شود و تمامی اطلاعات آنها محرمانه باقی خواهد ماند. از روش تجزیه و تحلیل موضوعی برای تحلیل داده‌ها استفاده گردید. یافته‌ها: در این پژوهش، براساس تحلیل موضوعی انجام شده 41 زیر موضوع و 3 موضوع اصلی استخراج شد. این سه موضوع اصلی شامل چالش‌های سازمان‌های بیمه‌گر، چالش‌های سیستم سلامت و چالش‌های بیمار بود. هر کدام از این موضوعات نیز دارای زیر موضوعاتی بودند. نتیجه‌گیری: نتایج این پژوهش چالش‌های ناشی از تقاضای القایی را ارائه می‌کند که بارزترین یافته‌های آن شامل چالش‌های سازمان‌های بیمه‌گر، چالش‌های سیستم سلامت و چالش‌های بیمار بود. این یافته‌ها به سیاستگذاران بخش سلامت کمک می‌کند تا بتوانند با درنظر گرفتن چالش‌ها، راهبردهای مناسبی در جهت کاهش آنها طراحی نمایند. واژه‌های کلیدی: مراقبت‌های بهداشتی؛ سیستم‌های مراقبت بهداشتی؛ بیمار؛ بیمه؛ هزینه‌های درمانی

Challenges Resulting from Healthcare induced Demand: a qualitative study

Introduction: Induced demand is one of the challenges of health care systems in different countries. It increases health care costs and also increases catastrophic cost index. This article seeks to examine the challenges of induced demand with the use of expert’s experiences of Isfahan University of Medical Sciences. Methods: The research is applied a qualitative method which done in Isfahan in 2012. Semi-structured interview was used for data gathering. For validity and reliability criteria such as data reliability of information and stability were considered. Participants in this study were people who had been informed in this regard and had to be experienced and were known as experts. Purposive sampling was done for data saturation. 17 people were interviewed. The anonymity of the interviewees was preserved. The data are transcribed, categorized and then used the thematic analysis. Results: In this study thematic analysis was conducted and 41 sub-themes and three themes were extracted. The three main themes include insurance organizations challenges, health systems challenges and patient’s challenges. Each of them has some sub-themes. Conclusion: the results of this study provide challenges due to induced demand. The most notable findings include insurance organizations challenges, health care system challenges and patient challenges. These findings will help health policymakers consider challenges to design appropriate strategies to reduce them. Keywords: Healthcare; Healthcare System; Insurance; Patients; Healthcare Costs
