چکیده
مقدمه: پرداخت مستقیم به عنوان یکی از منابع مالی نظامهای سلامت تأثیر منفی بر وضعیت سلامتی و شاخصهای کمی و کیفی عملکرد نظام سلامت دارد. هزینههای سلامتی بالاتر و در نتیجه پرداخت از جیب بیشتر ممکن است منجر به فقر یا کاهش دسترسی به خدمات سلامتی شود. متأسفانه این وضعیت بیشتر در میان افراد فقیر که نیاز بیشتری به مراقبت دارند دیده میشود. هدف این مطالعه جواب دادن به این مسأله بود که اگر سازمان تأمین اجتماعی بخواهد بیمهشدگانش را در برابر پرداخت مستقیم بالا ( صورت حسابهای بالای 1 میلیون ریال) حفاظت کند، با فرض ثابت بودن هزینههای کل چگونه پوشش هزینهی سایرین را تغییر دهد. روش بررسی: این پژوهش به صورت توصیفی و مقطعی بر روی صورت حسابهای بیماران بستری تحت پوشش بیمهی سازمان تأمین اجتماعی در بیمارستانهای شهر اصفهان در سال 1387 انجام شد. نمونهگیری به صورت خوشهای بود و براساس فرمول نمونهگیری طبقهبندی شده، 2430 عدد تعیین شد، سپس سهم هر بیمارستان و سهم هر بخش بستری مشخص گردید. اطلاعات از طریق مراجعه به واحد حسابداری و درآمد بیمارستان با مطالعهی فرمهای مربوط به هزینهی بیماران بستری و مصاحبه با کارشناسان بیمه در بیمارستان با استفاده از فرمهای رسمی اطلاعات امور مالی دفتر اسناد پزشکی جمعآوری شد. با توجه به استاندارد بودن فرمها، روایی و پایایی آنها مورد تایید بود. تحلیل دادهها با استفاده از نرمافزار Excelانجام شد. یافتهها: سازمان تأمین اجتماعی میتواند با سه سناریو با فرض ثابت بودن هزینههای سازمان، فرانشیزهای بالای یک میلیون را پوشش دهد. سناریوی اول: سازمان تأمین اجتماعی به جای پوشش 90 درصدی هزینهها برای پروندههایی که هزینههای درمان آنها کمتر از ده میلیون ریال باشد (فرانشیز کمتر از میلیون ریال) 43 درصد از هزینهها را پوشش دهد. سناریوی دوم: سازمان تأمین اجتماعی به جای پوشش 90 درصدی هزینهها برای پروندههایی که هزینههای درمان آنها تا بیست میلیون ریال است (فرانشیز کمتر از دو میلیون ریال) 77 درصد از هزینهها را پوشش دهد. سناریوی سوم: سازمان تأمین اجتماعی به جای پوشش 90 درصدی هزینهها برای پروندههایی که هزینههای درمان آنها کمتر ازصد میلیون ریال باشد (فرانشیز کمتر از ده میلیون ریال) 86 درصد از هزینهها را پوشش دهد. نتیجهگیری: سازمان تأمین اجتماعی با استفاده از فرانشیز متغیر میتواند افراد را در برابر هزینههای سلامتی بالا و خانوارها را در برابر هزینههای کمرشکن محافظت کند. واژههای کلیدی: بیمارستانها؛ تأمین اجتماعی؛ هزینهها

Survey Variable Copayment hospital bills for social security organization's insured patients in Isfahan Hospitals

Introduction: Out of pocket payments (OOP) as a kind of financial resources in health systems has regressive effects on health status, equity and both qualitative and quantitative health system performance indicators. Higher health expenditures and higher OOP may lead to more people fall in poverty or decrease their accessibility to health services. Unfortunately this could be found more among poor people whom need more protection. This study aimed to reveal if social security organization (SSO) wants to protect their insured people from higher amounts of OOP (bills more than 100 million Rials) how it would affect cost coverage of other people assuming fixed total cost of all bills for SSO. Methods: That was a cross sectional study on Isfahan hospital bills for inpatients of SSO during 2008. Sampling method was cluster sampling. We studied 2430 hospital bills and copayment paid directly by patient. The data were collected by Finance information form. The form was a standard so it is the reliable and valid. Data analyzed with Excel software and descriptive statistics. Results: Now SSO covers around 90% of hospital bills for its insured patients regardless of bill amount. Our calculations according to before mentioned show that SSO should pay less to low cost bills through one of these scenarios: First: for all bills fewer than 10 million Rials, covering 43% of costs. Second: for all bills fewer than 20 million Rials, covering 77% of costs. Third: for all bills fewer than 100 million Rials, covering 86% of costs. Conclusion: Findings show that social security organization by using Variable Copayment in different manners could protect individuals with high health expenditures and protect households against catastrophic costs.Key words: Hospitals; Social Security; Costs
