چکیده
چکیده یکی از اهداف تدابیر مراقبتهای بهداشتی و درمانی و برنامههای مربوط به آنها، ارتقاء و بهبود شرایط زندگی است. مناسبترین هدف برای خدمات بهداشتی و درمانی دسترسی یکسان مردم به خدمات ذیربط می باشد. یکی از موانع دسترسی، بالا بودن هزینههای خدمات درمانی و بار مالی این هزینهها بر دوش خانوار است. نگرانی در مورد افزایش هزینههای مراقبتهای بهداشتی و درمانی و پیدا کردن راه حل برای رفع آن باعث شده است که سیاستگذاران توجه وافری به یک سیستم تأمین مالی ملی برای کاهش بار مالی خانوارها متمرکز کنند. این پژوهش تاثیر هریک از عوامل تعیینکننده پرداختی از جیب را با استفاده از مدل گسسته دو مرحلهای هکمن بررسی کرده است. برای این منظور داده‌ها به روش میدانی و پرسشنامهای میباشد نمونه مورد مطالعه شامل312 بیمار بستری در بیمارستانهای شهر تبریز است. نتایج نشان داد که درآمد، سن، خصوصی بودن بیمارستان، دارای بیماری زمینهای بودن، روستایی بودن تأثیر مثبت و دارا بودن بیمه تأثیر منفی بر میزان هزینههای پرداختی از جیب دارند و خصوصی بودن بیمارستان، دارای بیماری زمینهای بودن، تأثیر مثبت و معنیدار و بومی بودن بیمار و نوع پذیرش تأثیر منفی و معنیداری بر احتمال وجود OOP دارند. علیرغم بیمعنی بودن متغیرهای سن و درآمد به دلیل بالا بودن t آنها در مدل باقی میمانند که تأثیر هر دو بر احتمال وجود OOP مثبت میباشد.کلید واژه : هزینه ها؛ ترخیص بیمار؛ بیماران؛ بیمه.

چکیده [English]
Introduction:The most important goals of the health and cure services are promotion of life quality and equal access of all the people to aforementioned services. Higher health care costs and its financial pressures upon households' shoulders is one of the barriers of equal access to these services that policy-makers and academics have concerned about these rising costs.Methods and material: Using Heckman 2 stages model this research attempts to find out the effects of the factors cause to the out of pocket payment. The data is collected through a questionnaire.To do so, a sample of 312 patients admitted to hospitals in the city of Tabriz has been surveyed. Results:The results indicate that, income, age, being in private hospital, ruraland having an underlying diseasehave positive impact on the out of pocket cost. Also it found that, being in a private hospitals and having an underlying disease significantly have positive effect (due to greater than 2 t-statistic), and being a local patient and also type of admission, have significant negative impact on out of pocketConclusion:Income, age, being a private hospital, rural and has an underlying disease has positive impact on the out of pocket cost. So the government extending the insurance coverage for People and do the appropriate action forReduce direct payments out of pocket in the elderly and non-native Persons.Key words: Costs; Patient Discharge;Patients; Insurance
