چکیده

مقدمه: اجرای مدیریت مطلوب داده‌ها، برای هر کارآزمایی بالینی ضروری است. با این حال ممکن است این مسأله در طول مراحل برنامه‌ریزی و اجرای کارآزمایی بالینی نادیده گرفته و مورد غفلت واقع شود. هدف از انجام این مطالعه، شناسایی وضعیت مدیریت داده‌های کارآزمایی‌های بالینی در مراکز تحقیقاتی از مرحله‌ی برنامه‌ریزی تا ذخیره‌ی آن بر اساس استاندارد بین‌المللی بود.
روش بررسی: مطالعه از نوع توصیفی- تطبیقی و مقطعی بود که در سال 1390 انجام شد. مطالعه کارآزمایی بالینی دارویی که در مراکز تحقیقاتی درمانی دانشگاه علوم پزشکی انجام شد، جامعه‌ی پژوهش این مطالعه بود. 42 مرکز تحقیقاتی بر اساس امتیاز ارزشیابی وزارت بهداشت به سه گروه قوی، متوسط و ضعیف تقسیم شدند. از هر گروه مراکز تحقیقاتی، 15 کارآزمایی بررسی شد. کلیه‌ی مدارک و مستندات و هر نوع منبعی در رابطه با کارآزمایی‌های بالینی مورد بررسی قرار گرفت. ابزار جمع‌آوری داده‌ها، چک لیست شامل 239 سؤال بود که بر اساس استاندارد ‌مدیریت مطلوب داده‌های بالینی تدوین شده بود. جهت سنجش روایی محتوای، چک لیست بین ده نفر از متخصصان مربوطه توزیع گردید. برای تحلیل داده‌های جمع‌آوری شده از آزمون های Mann-Whitney U و One-Way ANOVA استفاده شد.
یافته‌ها: تفاوت معنی‌دار آماری بین میانگین گروه‌های مراکز تحقیقاتی قوی، ضعیف و متوسط وجود داشت. همچنین میانگین در دو گروه مراکز قوی و متوسط برای اهداف مربوط به گزارش‌دهی و کنترل کیفیت داده‌ها با یکدیگر یکسان بود، اما با مراکز ضعیف تفاوت داشتند. بالاترین میانگین به دست آمده، مربوط به نحوه‌ی گزارش‌دهی داده‌ها بود که به ترتیب در مراکز قوی، متوسط و ضعیف 82 درصد، 81 درصد و 75 درصد بوده است. همچنین کمترین میانگین مربوط به کنترل کیفیت داده‌ها به مقادیر 09/0 درصد، 07/0 درصد و 0 درصد در مراکز قوی، متوسط و ضعیف بوده است.
نتیجه‌گیری: مدیریت داده‌های کارآزمایی‌های ‌بالینی که توسط مراکز تحقیقاتی انجام شده است در تطابق با استاندارد بین‌المللی از وضعیت مطلوبی برخوردار نبود. بنابراین، ایجاد استانداردهای ملی مدیریت داده با در نظر گرفتن جزئیات فنی، تشکیل کمیته‌های نظارت و کنترل کیفیت داده‌ها و نیز آموزش اصول مدیریت داده‌ی مطلوب به مجریان و کلیه‌ی همکاران مطالعات کارآزمایی بالینی جهت برآورده کردن انتظارات ملی و بین‌المللی و همچنین دستیابی به داده‌های با کیفیت و معتبر در ایران ضروری به نظر می‌رسد.

Introduction: Good data management practices are essential to any clinical trial, yet this area is one of
which can be neglected during the planning stages of a trial. This study focused on the management of the
data during each stage of the life for a trial from planning to archive in compliance with good clinical data
management practice (GCDMP) standard in clinical trials conducted by the clinical research centers.
Methods: This cross-sectional study about data management of clinical trials in clinical research centers
was conducted in 2010. A checklist containing 239 questions for total objectives was developed using the
standards described in the GCDMP. Based on the Ministry of Health evaluation scores, clinical research
centers were divided into three categories of strong, average, and weak. We reviewed documents,
facilities, records and any other resources of 15 clinical trials for each category. Collected data were
analyzed by Mann-Whitney U and analysis of variance (ANOVA) tests using SPSS software.
Results: Results showed that there was a statistically significant difference between strong, average, and
weak groups. In terms of data quality and reporting, no statistically significant difference was indicateed
between strong and average clinical trials groups.
Conclusion: Clinical trial data management in Iran did not meet GCDMP requirements. Therefore,
development of national specific and practical standard with technical detail, establishing a data quality
monitoring committee, clinical research assistant training for GCDMP requirements are necessary for
achieving data quality and international expectations.
