ارزیابی سیستم اطلاعات بیمارستان ده بیمارستان دولتی زیر نظر دانشگاه علوم پزشکی اصفهان

چکیده
مقدمه: سیستم اطلاعات بیمارستان نقش مهمی در افزایش کیفیت درمان در بیمارستان‌ها دارد. سیستم اطلاعات بیمارستان به عنوان نظام اطلاعاتی تلقی می‌شود که می‌تواند تأثیر فراوان بر اثربخشی و کارایی مدیریت بیمارستان داشته باشد. نظام ارزیابی به عنوان یکی از مباحث مهمی است که در نظام مدیریت مورد تأکید فراوان قرار گرفته است. در این مقاله به ارزیابی سیستم اطلاعات بیمارستان (HIS یا Hospital information system) بر اساس مدل Ahitof and Newman پرداخته شده است و هدف کلی این پژوهش، ارزیابی سیستم اطلاعات بیمارستان‌هایی بود که از نرم‌افزار کوثر (نسخه‌ی جدید) استفاده می‌کردند.
روش بررسی: تحقیق حاضر از نظر هدف، کاربردی و از نظر روش، توصیفی و پیمایشی از شاخه‌ی‌ میدانی بود که در سال 1391 انجام گرفت و ابزار جمع‌آوری اطلاعات داده‌های آن آمیزه‌ای از ابزار کتابخانه‌ای و میدانی بود. جامعه‌ی آماری این پژوهش، ده بیمارستان بود که زیر نظر دانشگاه علوم پزشکی بودند و همه از سامانه‌ی کوثر یاری گرفتند. جامعه‌ی آماری این پژوهش 209 نفر از اپراتورهای شیفت صبح بودند که با توجه به محدود بودن حجم جامعه، نیازی به نمونه‌گیری نبود و کل جامعه از طریق سرشماری مورد بررسی قرار گرفت. در این پژوهش از پرسش‌نامه‌ی استاندارد Ahitof and Newman استفاده شد و عملیات بومی‌سازی انجام گرفت. پرسش‌نامه شامل 37 سؤال بر اساس طیف پنج گزینه‌ای لیکرت بود. روایی محتوایی سؤالات پرسش‌نامه از طریق پانل نخبگان (از 5 نفر از اساتید متخصص در سیستم‌های اطلاعاتی مدیریت و مسؤولین HIS با سابقه‌ی بالا) تأیید شد و پایایی آن نیز از طریق Alpha Cronbakh، 85/0 محاسبه گردید (در صورتی که ضریب پایایی از 75/0 بیشتر می‌شد مورد قبول بود). جهت تحلیل داده‌ها از نرم‌افزار SPSS استفاده شد.
یافته‌ها: نتایج مربوط به پرسش‌نامه نشان داد که از سه فرضیه‌ی پژوهش (یعنی وجود ابعاد زمانی، محتوایی و ساختار) دو فرضیه یعنی ابعاد زمانی و محتوا در سطح اطمینان 95/0 مورد تأیید قرار گرفتند و در سطح ساختار در حد مطلوبی نبوده است. 
نتیجه‌گیری: HIS حاکم بر سازمان از لحاظ به موقع بودن، به هنگام بودن، صحت، مرتبط بودن، ایجاز و تناسب در وضعیت مطلوبی بوده اما از لحاظ بسامد، دوره‌ی زمانی، جامعیت در حد مطلوبی نبود و نیاز به ارایه‌ی راه‌حل‌هایی برای آن‌ها است. نیاز است سیستم اطلاعات بیمارستان از لحاظ ساختاری مورد بازنگری و تجدید نظر قرار گیرد و نیاز است که اطلاعات با جزییات بیشتر و در قالب نمودارهای آماری متفاوت ارایه گردد و نیز نسخه‌ی سوم نرم‌افزار کوثر با اصلاحات جدید ارایه گردد.

چکیده English

Introduction: Hospital information system (HIS) plays an important role in enhancing the quality of
remedy in hospitals. HIS is considered as an information system that may have great influence on the
effectiveness and efficiency of hospital management. Evaluation system is one of the most important
issues that is greatly emphasized in the management system. This paper evaluate HIS based on Ahituv and
Neumann model. The general objective of this study was to assess HIS in hospitals where Kowsar
software is in use.
Methods: This study used a descriptive-field survey approach and carried out in 2012. The data were
collected via both field and library sources. Subset of this research contained ten governmental hospitals
under supervision of Isfahan University of Medical Sciences, Iran. Indeed, the Kowsar software was
installed in all the studied hospitals. Since the number of subjects was 209, it was feasible to use all staff.
The volunteers were given localized questionnaires based on Ahituv and Neumann model indices (time,
content, and structure) to assess the HIS. The questionnaire included 37 items with regard to the fivechoice
likert scale. The elite panel confirmed questionnaire content validity. In addition, reliability was
calculated via Cronbach’s alpha (0.85).
Results: Data analysis showed that there were two true hypotheses among the three. In fact, both time and
content were verified with 0.95 reliability rate but the structure was not in an acceptable level.
Conclusion: The HIS governing the organization was in a proper condition in terms of timeline, accuracy,
relevancy, and appropriateness of brevity; however, in terms of frequency, period, and universality it was
not satisfactory and needed modification. HIS basically requires restructuring and also detailed
information and several figures and charts to illustrate statistical outputs. Altogether, a newer version of
Kowsar can be released to overcome the drawbacks.
