اولویت‌بندی عوامل مرتبط با پذیرش فناوری بلاک‌چین در نظام پرونده الکترونیک سلامت

چکیده
مقدمه: فناوری بلاک‌چین به عنوان چهارمین انقلاب صنعتی در سال‌های اخیر ظهور کرده که از طریق ارائه یک زیرساخت جدید می‌تواند مدیریت و تبادل سوابق بیماران را تسهیل نماید. مرور ادبیات حاکی از کمبود مطالعات پیرامون عوامل مرتبط با پذیرش این فناوری در صنعت مراقبت‌های بهداشتی می‌باشد. بنابراین، هدف مطالعه حاضر، اولویت‌بندی عوامل مرتبط با پذیرش فناوری بلاک‌چین در نظام پرونده الکترونیک سلامت (Electronic Health Record: EHR) می‌باشد.

روش بررسی : در این پژوهش توصیفی، ابتدا با مروری بر متون علمی 15 عامل مرتبط با پذیرش بلاک‌چین در سیستم‌های EHR شناسایی شد. سپس این عوامل توسط 17 خبره که با روش نمونه‌گیری هدفمند انتخاب شده بودند، اعتبارسنجی شده و در پنج دسته: فناورانه، حقوقی، مالی، محیطی و سازمانی تقسیم شدند. در نهایت وزن هر معیار با روش بهترین-بدترین فازی تعیین و سازگاری نظرات خبرگان ارزیابی گردید.

یافته‌ها: با توجه به قضاوت‌های خبرگان، معیار حقوقی با 32 درصد به عنوان مهم‌ترین عامل در پذیرش فناوری بلاک‌چین در سیستم‌های EHR شناسایی شد. علاوه بر این، امنیت و حریم خصوصی ( 66/16 درصد)، رعایت الزامات قانونی (10/13 درصد)، انطباق‌پذیری با تغییرات در قوانین (22/12 درصد)، مشوق‌ها و پاداش‌ها (01/7 درصد) و استانداردسازی (87/6 درصد) به ترتیب به عنوان پنج زیرمعیار مهم در پذیرش این فناوری تشخیص داده شدند.

نتیجه‌گیری: به کمک عوامل شناسایی شده در این مطالعه و وزن‌های تعیین شده برای هر یک از آن‌ها می‌توان روشی برای ارزیابی سطح آمادگی مراکز ارائه‌دهنده خدمات مراقبت‌های بهداشتی در پذیرش فناوری بلاک‌چین در سیستم‌های EHR توسعه داد.

Prioritization of the Factors Related to the Adoption of Blockchain Technology in the Electronic Health Record Systems

Introduction: Blockchain technology has emerged as the fourth industrial revolution in recent years, which can facilitate the management and exchange of patient records by developing a new infrastructure. A review of the literature indicates the lack of studies and the need for further study of the factors affecting the adoption of this technology in the healthcare industry. Therefore, the aim of the present study is to prioritize the factors related to the adoption of blockchain technology in the Electronic Health Record (EHR) systems.

Methods: In this descriptive study, 15 factors related to blockchain adoption in EHR systems were identified by reviewing the scientific literature. Then, these factors were validated by 17 experts who were selected by targeted sampling method and divided into five categories: technological, legal, financial, environmental and organizational. Finally, the weight of each criterion was determined by the fuzzy best-worst method and the consistency of experts’ opinions was evaluated.

Results: According to the judgments of experts, the legal criterion with 32 percent was identified as the most important factor in the adoption of blockchain technology in EHR systems. In addition, security and privacy (16.66 percent), compliance with legal requirements (13.10 percent), compliance with changes in laws (12.22 percent), incentives and rewards (7.01 percent) and standardization (6.87 percent) were identified as five most important sub-criteria in the adoption of the technology, respectively.

Conclusion: With the help of the factors identified in this study and the weights determined for each of them, a method can be developed to assess the level of readiness of healthcare providers in adopting blockchain technology in EHR systems.
