چکیده
مقدمه: سیستم‌های بهداشتی در مدیریت حوادث غیر مترقبه با چالش‌های منحصر به فردی روبرو است که از جمله‌ی آن‌ها تریاژ و درمان تعداد وسیعی از مصدومان و نیز مدیریت اطلاعات آنان می‌باشد. تضمین مستندسازی مصدومان به شیوه‌ای صائب به عنوان مقدمه‌ی مدیریت اطلاعات و تریاژ و درمان کارآمد آنان مستلزم پروندهی پزشکی مناسب و قابل استفاده در حوادث غیر مترقبه است. زیرا پروندهی پزشکی که در حالات عادی برای بیماران مورد استفاده واقع می‌شود، در موارد اضطراری و وقوع حوادث مؤثر نبست و کاربردی پیچیده خواهد داشت. بنابراین هدف از انجام پژوهش حاضر طراحی پروندهی پزشکی مصدومان حوادث غیر مترقبه است که به وسیله‌ی آن بتوان در هنگام وقوع حوادث به سهولت و سرعت به مستندسازی اطلاعات و تریاژ مصدومان پرداختروش بررسی: مطالعه‌ی حاضر توصیفی و از نوع کاربردی است. محیط انجام پژوهش کتابخانه‌ها، مقالات تأیید شده در بانک‌های اطلاعاتی و منابع اینترنتی بود. طی بررسی سازمان‌های مسؤول رسیدگی به مصدومان حوادث غیر مترقبه در اصفهان محرز شد که در حال حاضر پروندهی پزشکی برای مستندسازی وضعیت مصدومان وجود ندارد. بنابراین ابتدا مراکز مختلفی که در دنیا مسؤول گردآوری و سازماندهی اطلاعات مصدومان حوادث غیر مترقبه‌اند مشخص شدند و ضمن مطالعه‌ی تجربیات آن‌ها، داده‌های مورد نیاز پروندهی پزشکی در حوادث غیر مترقبه از متون علمی چاپی و الکترونیکی جمع‌آوری گردید؛ سپس پارامترهای ضروری پروندهی پزشکی در حوادث غیر مترقبه با بهره‌گیری از 2 الگوی آمریکایی و اروپایی استخراج شدیافته‌ها: با توجه به شرایط و استانداردهای کشوری، الگوی مناسبی برای پروندهی مصدومان حوادث غیر مترقبه در دو محور ویژگی‌های فیزیکی و عناصر اطلاعاتی با همکاری مرکز مدیریت حوادث و فوریت‌های پزشکی استان اصفهان طراحی و ارایه گردیدنتیجه‌گیری: از آن‌جا که در عملیات امداد حوادث غیر مترقبه پروندهی پزشکی باید ساده، گویا و قابل استفاده باشد، پروندهی پزشکی که در حالت عادی برای بیماران در بیمارستان‌ها استفاده می‌شود برای مصدومان حوادث غیر مترقبه کارآیی ندارد. در الگوی پیشنهادی، پروندهی طراحی شده فرمی ساده و گویا است که در زمان بروز حادثه امکان مستندسازی اطلاعات مصدومان را فراهم می‌سازد و مرجعی سریع برای دسترسی به مهم‌ترین اطلاعات فوریتی بالینی، اجرایی و آماری حوادث غیر مترقبه خواهد بودواژه‌های کلیدی: بلایا؛ تریاژ؛ مدارک پزشکی؛ مدیریت ایمنی از حوادث؛ مدیریت اطلاعات.

Introduction: Disaster occurs almost daily in the world and increases the issue of it because of high volume of population, industrialization and acts of terrorism, my country is one of the most unexpected event areas of the world. Health care systems encounter special challenges in disaster management, that they include triage and treatment a large number victims and also victims' information management. To ensure correct way of documentation of victims as introduction information management and effective triage and treatment, useful medical record are necessary, because medical record usually used for patients are too complicated and ineffective to use in case of emergency.So the objective of this research is design the disaster-victim medical record to document and triage easily for victims in disaster.Methods: This research is an applied study and has been performed as descriptive study. Source of information is libraries and accepted articles in indexed journals. During survey of corresponding organizations about disaster in Isfahan and absence a medical record for documentation of victims, researchers identified the organizations of disaster in the world and then gathered necessary data elements for medical record in disaster. Then necessary parameters for the medical record extracted from American and European models.Results: According to situation and standards of Iran, a proper and final pattern designed with assisting management and medical emergencies center in Isfahan province.Conclusion: Medical record in disaster relief operations must be simple and useful; medical record used in hospitals isn't effective. The proposed model is simple and proper form for documentation of victims and easy reference for accessing clinical, administrative and statistics information in disaster.Keywords: Disasters; Triage; Medical Record; Safety Management; Information Management.
