چکیده

مقدمه: سازمان جهانی بهداشت با ایجاد تغییرات در ساختار سیستم طبقه‌بندی بین‌المللی بیماری‌ها سعی در روزآمدسازی آن متناسب با نیازهای اطلاعاتی جامعه‌ی علمی دارد. در این مطالعه تغییرات 10-ICD (International classification of diseases) در فرایند روزآمدسازی مورد بررسی قرار گرفت.
روش بررسی: داده‌های این مطالعه‌ی توصیفی برگرفته از سایت سازمان جهانی بهداشت بود که پس از دسته‌بندی در قالب تعیین نوع تغییرات، نوع دستورالعمل‌ها جهت ایجاد تغییر، منبع ایجاد تغییر و سال تأیید و اعمال تغییرات بر اساس آمار توصیفی مورد تفسیر
قرار گرفت.
یافته‌ها: از تعداد 1914 تغییری که در 10-ICD در فاصله‌ی سال‌های 1995 تا 2009 رخ داده است، بیشترین تغییر در جلد سوم با 1249 مورد (53/65 درصد) بوده است. بیشترین تغییرات بزرگ در جلد اول با 264 مورد (19/50 درصد) و بیشترین تغییرات کوچک در جلد دوم با 69 مورد (64/49 درصد) اعمال شده است. بیشترین دستورالعمل تغییرات در هر سه جلد مربوط به اضافه کردن (Add) موردی به سیستم بوده است. در بین گروه‌های فعال ایجاد کننده‌ی تغییرات، MRG (Mortality reference group) با 107 مورد (34/20 درصد) و 116 مورد (45/83 درصد) به ترتیب در جلد اول و دوم و آلمان با 305 مورد (42/24 درصد) در جلد سوم بیشترین نقش را داشته‌اند.
نتیجه‌گیری: حفظ سیالیت یک سیستم طبقه‌بندی در حوزه‌ی سلامت امری خطیر و ضروری است. این سیالیت در 10-ICD با ایجاد تغییرات چشم‌گیر به دلیل افزایش کشفیات جدید در زمینه‌ی پزشکی، تجهیزات تشخیصی و درمانی و به دنبال آن اطلاعات سلامت رخ داده است. در ایران نیز به دلیل سیاست دولت مبنی بر ایجاد پرونده‌ی الکترونیک سلامت ایرانیان، نیاز به استفاده از سیستم طبقه‌بندی روزآمد بیشتر احساس می‌شود.

Introduction: World Health Organization (WHO) attempts to update ICD-10 (international
classification diseases-10th version) according to the required information of the scientific
community. In this study, changes of ICD-10 will be reviewed in updating process.
Methods: The data of the present descriptive study was extracted from the World Health
Organization website. The data were interpreted based on descriptive statistics after classification
of changes, type of instructions to make change, source of changes, and approved and
implementation date of the changes.
Results: Out of 1914 changes occurred in ICD-10 during 1995 and 2009, the greatest change
happened in the third volume with 1249 cases (65.5%). Most of the major changes were in the first
volume with 264 cases (50.1%) and most of the minor changes were in the second volume with 69
cases (49.6%). The majority of the instruction changes were adding an item to the system in every
three volumes. Among the active groups for creating modification, mortality reference group
(MRG) had the highest role in the first and second volumes with 107 (20.3%) and 116 cases
(83.4%), respectively. Germany with 305 cases (24.4%) in the third volume had the highest role.
Conclusion: Maintaining the fluidity of a classification system in the field of health is an
enormous and necessary task. This fluidity has occurred in ICD-10 with making significant
changes due to new discoveries in the field of medicine, diagnostic and therapeutic equipments
and health information subsequently. In Iran, due to the governmental policy to create the Iranian
electronic health records, the updated health classification is further needed.
