میزان آمادگی بیمارستان‌های دارای سیستم مدیریت کیفیت اصفهان بر اساس مدل عملکردی استاندارد اعتباربخشی کمیسیون مشترک بین‌الملل

چکیده
مقدمه: امروزه موضوع اعتباربخشی با هدف ایجاد و ارتقای فرهنگ ایمنی و کیفیت مراقبت از بیمار جایگاه ویژه‌ای یافته است. در چند سال اخیر، اقداماتی نیز در جهت استقرار استاندارد اعتباربخشی در کشورمان در جریان بوده است که در سال 1390 جایگزین استانداردهای ارزشیابی بیمارستان گردید. هدف پژوهش حاضر، تعیین میزان آمادگی بیمارستان‌های منتخب استان اصفهان بر اساس مدل عملکردی استاندارد اعتباربخشی کمیسیون مشترک بین‌الملل بود.
روش بررسی: مطالعه‌ی توصیفی- مقطعی حاضر در سال 1390 بر روی چهار بیمارستان دکتر غرضی، دکتر شریعتی، فاطمه الزهرا (س) نجف‌آباد و شهید بهشتی صورت گرفت. ابزار جمع‌آوری داده‌ها، 13 چک‌لیست مدل عملکردی کمیسیون اعتباربخشی بین‌المللی با 323 گزاره‌ی مورد تأیید مرکز نظارت و اعتباربخشی معاونت درمان وزارت بهداشت، درمان و آموزش پزشکی بود. پژوهشگر از طریق مشاهده‌ی مستندات و پرسش از نمایندگان مدیریت کیفیت بیمارستان، داده‌ها را جمع‌آوری و بر اساس اهداف پژوهش در سطح آمار توصیفی و در نهایت از طریق نرم‌افزار SPSS مورد تحلیل قرار داد.
یافته‌ها: میزان آمادگی بیمارستان‌های منتخب در استانداردهای بیمار محور حدود 64 درصد و در استانداردهای سازمان محور حدود 66 درصد بود. بیمارستان‌های مورد بررسی در راستای استقرار استانداردهای اعتباربخشی با میانگین حدود 65 درصد دارای آمادگی نسبی بودند.
نتیجه‌گیری: با توجه به این که بیمارستان‌های مورد مطالعه دارای نظام مدیریت کیفیت (9000 ISO یا
International Organization for Standardization 9000) بود، امکان استقرار استانداردهای نوین ارزیابی فرایند، پیامد و ساختاری به جای استانداردهای سنتی در این بیمارستان‌ها وجود دارد. در این راستا، برخی از محورهای مورد بررسی، از جمله حقوق بیمار باید به طور جدی مورد توجه متولیان نظام سلامت قرار گیرد.

Introduction: Recently, the accreditation program has been used increasingly by an emphasis on
patient safety. In Iran, efforts have been made to establish the accreditation program. In 2011, it
replaced previous standards of traditional evaluation system. As the impact of accreditation plan
need internal self assessment against minimal standards, this study aimed to survey the readiness
of selected hospitals in Isfahan based on the functional model of Joint Commission on
Accreditation.
Methods: This cross-sectional descriptive study was conducted on four hospitals in Isfahan, Iran.
Data was collected using 13 checklists based on the functional model of Joint Commission
International Accreditation. The checklists were verified by the Iranian Ministry of Health. Data
was collected via observations and interviews with hospital representatives in quality
management. All statistical analyses were performed using SPSS.
Results: The readiness of the selected hospitals in Isfahan was 64% regarding patient-based
standards and 66% for facility-based standards. In general, the studied hospitals had adequate
readiness for implementation of accreditation standards.
Conclusion: As the study population has ISO 9000 certification, it is possible to replace old
methods of process, outcome, and structure evaluation with new standards. Accordingly, patient
rights have to be considered by health care employers in Isfahan.
