ارزیابی شاخص توزیع پزشکان متخصص در بیمارستان‌های دولتی ایران

چکیده
مقدمه: عدالت در سلامت به معنای عدالت در توزیع تندرستی جامعه است. منظور از رعایت عدالت در سلامت، توجه به نیازهای سلامت همه‌ی افراد و اقشار جامعه بدون توجه به موقعیت جغرافیایی، فرهنگی، اقتصادی و اجتماعی آن‌ها است. هدف از پژوهش حاضر مطالعه‌ی برآورد توزیع نیروی انسانی پزشک متخصص به تفکیک رشته‌های تخصصی بر حسب جمعیت (000,100 نفر) در بیمارستان‌های دولتی استان‌های کشور بود.
روش بررسی: این پژوهش مقطعی- توصیفی بود که بر روی اطلاعات جمع‌آوری شده‌ی پزشکان متخصص در سال 1389 انجام گرفت. در این مطالعه، تعداد متخصصان پزشک کشور بر اساس فرم‌هایی که توسط وزارت بهداشت، درمان و آموزش پزشکی به معاونت درمان کلیه‌ی دانشگاه‌های علوم پزشکی کشور ارسال شده بود، جمع‌آوری گردید. همچنین جمعیت کشور از وب‌سایت مرکز آمار ایران استخراج گردید و روایی و پایایی همه‌ی اطلاعات تأیید شده بود. شاخص‌های مرکزی، شامل بیشترین و کمترین انحراف معیار و میانگین تعداد متخصصان در بین استان‌های کشور محاسبه گردیدند. نرخ داده‌ها بر حسب نوع تخصص به ازای 000,100 نفر جمعیت نیز محاسبه شد. برای نمایش ارتباط تعداد نیروهای متخصص با جمعیت، از نمایش ضریب همبستگی میان آن‌ها استفاده گردید. داده‌ها با استفاده از نرم‌افزار SPSS نسخه‌ی 16 مورد تجزیه و تحلیل قرار گرفت.
یافته‌ها: بیشترین و کمترین شاخص متخصص به 000,100 نفر جمعیت در رشته‌های تخصصی در رشته‌ی اطفال با شاخص 72/1 و جراحی مغز و اعصاب با 31/0 مشاهده شد. همچنین بالاترین میزان همبستگی میان رشته‌های تخصصی و جمعیت در رشته‌ی ‌پاتولوژی به میزان 96/0 و کمترین میزان در رشته‌ی روانپزشکی به میزان 47/0 بود. همچنین در کل، بیشترین و کمترین شاخص پزشک در رشته‌های تخصصی در استان‌های کشور به ترتیب استان یزد با شاخص 22/19 و استان سیستان و بلوچستان با شاخص 8/8 بود.
نتیجه‌گیری: همبستگی بالایی میان تعداد پزشک متخصص به تفکیک رشته و جمعیت (000,100 نفر) در بیمارستان‌های دولتی کشور وجود داشت. همچنین کاهش میزان انحراف معیارها، بیانگر همگن‌تر شدن پراکندگی متخصصین نسبت به 5 سال گذشته است که با اعمال و تداوم مدیریت صحیح می‌توان آن را بهبود بخشید

چکیده English

Introduction: Equity in health means justice in the distribution of community health. The
purpose of justice in health is to consider health needs of all people regardless of their
geographical location or cultural, economic, and social status. The aim of this study was to assess
distribution of specialists based on population (100,000 people) in public hospitals of Iranian
provinces during 2010.
Methods: In this descriptive study, distribution of each specialization in different provinces was
shown as maximum, minimum, mean, standard deviation, and median. In order to display the
relation between number of specialists and population, the correlation coefficient was employed.
Data was analyzed using SPSS.
Results: The highest and lowest ratios of specialists to population of 100,000 people in specialty
fields were observed in case of pediatricians (1.72) and neurosurgeons (0.31), respectively. The
highest and lowest correlations between specialized fields and population were in pathology
(0.96) and psychiatry (0.47), respectively. Meanwhile, the highest and lowest ratios of specialists
to population were observed in Yazd Province (19.22) and Sistan and Baluchestan Province (8.8),
respectively.
Conclusion: This study showed a high correlation between the number of specialists and
population in public hospitals of Iran. Moreover, distribution of specialists was more
homogeneous during the last five years. Appropriate management can improve the current status.
