چکیده

هدف نهایی نظام ارایه‌ی خدمات بهداشتی و درمانی هر کشوری، ارتقای سطح سلامت آحاد مردم می‌باشد. عدم پاسخگویی به نیازها و انتظارات سلامتی در چند دهه‌ی اخیر، اجرای برنامه‌های اصلاحی را ایجاب می‌کند. پزشک خانواده‌ی روستایی از جمله این اصلاحات در نظام سلامت ایران است که از سال 1384 به اجرا درآمد. هدف از این مطالعه، بررسی عملکرد برنامه‌ی پزشک خانواده‌ی روستایی از زمان آغاز این برنامه بود. مطالعه‌ی حاضر یک مطالعه‌ی مروری نقلی بود که با جستجو در سایت‌های Google، Google scholar، SID، Magiran و Irandoc با کلید واژه‌های «پزشک خانواده، پزشک خانواده‌ی روستایی، Family physician،
Family medicine،Rural family medicine ، Rural family physician» انجام گرفت که در نهایت 39 مقاله‌ی مرتبط با هدف مطالعه، استخراج و بررسی شد. در این مطالعه اهداف برنامه‌ی پزشک خانواده در چندین زیر مجموعه شامل نظام ارجاع، پاسخگویی، دسترسی، هزینه، پوشش خدمات، اولویت به خدمات سلامت‌نگر، تشکیل پرونده‌ی سلامت، کنترل کیفیت خدمات، رضایتمندی و نگرش کارکنان تیم سلامت و گیرندگان خدمت دسته‌بندی شد و عملکرد برنامه‌ی پزشک خانواده در دستیابی به این اهداف بررسی گردید. مطابق بررسی‌های انجام شده، برنامه‌ عملکرد به نسبت خوبی پس از گذشت چند سال از اجرا داشت، ولی هنوز نظام ارجاع و ارایه‌ی پسخوراند از پزشکان متخصص به سطح یک، پیگیری عاقبت بیماران و تکمیل پرونده‌ی سلامت به درستی اجرا نمی‌شود. دسترسی تا حدودی بهبود یافت و هزینه‌های غیر ضروری کاهش پیدا کرد، با این حال هنوز بینش سلامت‌نگر مطلوب در اذهان نهادینه نشده است.

چکیده [English]

The ultimate goal of the health care system of any country is to promote public health. Reforming
programs were required because of failure to respond to the needs and expectations of health in recent
decades. Rural family physician was a part of these reforms of the health system, which was implemented
in 2006. The aim of this study was to evaluate the performance of the rural family physician program
since it was started. This review was done by searching in Google, Google scholar, SID, Magiran,
Iran-doc with the following keywords: Family physician, rural family physician, family medicine, rural
family medicine. 32 related articles were extracted and analyzed. The objectives of family physician
program were classified in several sub-goals series including referral system, accountability, access, cost,
coverage, priority-based health services, formation of health record, control the quality of services,
satisfaction and attitudes of health team staff and service recipients and the family physician performance
in achieving this goals. Generally, after a few years of implementation, the program has performed
relatively well but the referral system and presenting feedback from specialists, patients follow-up and
completing health record was not implemented properly. Access was improved and unnecessary costs
were reduced although good holistic health insight has not institutionalized yet.
